What Would Ketones In A Urine Test Indicate?

Question: Im going to get my DOT Physical on Monday for truck driving school. Im afraid they're going to find ketones in my urine "sugar and protein" which would be a sign of diabetes. My mom who has type2 has been taking my blood sugar and it was 125 after eating at its highest and 86 at its lowest when fasting. Im 99.9% positive Im not diabetic and I think Im getting ketone readings on my ketostix because I have been starving myself over the last 2 days and frankly nearly starving over the last year to lose weight. Therefore my body is burning fat since Im eating only one meal a day at dinner causing ketones to appear in my urine. I would like a professional opinion though. I'm 22 6 feet tall a smoker and weigh just under 240 "down from 255."

Thanks for your question.

After going through your question, I can understand your concern.

You are at high risk of developing Type 2 diabetes as you have family history of diabetes and has Level I obesity (BMI 33.5).

Your capillary blood glucose readings taken with glucometer are not in diabetic range.

But by XXXXXXX Diabetic Association (ADA) criteria for diagnosis of diabetes is by doing OGTT (Oral Glucose Tolerance Test), HbA1c or Random venous blood glucose level in symptomatic patient and not by doing Capillary blood glucose level, urine sugar or ketones.

There may be different causes for presence of sugar, protein and ketones in urine, Diabetes is one them.
ie. presence of urine sugar, ketones, protein may require further evaluation for detection of cause.
And presence or absence of diabetes can be easily confirmed by doing OGTT or HbA1c.

Though you are starving possibility of detection of urine ketones is less as you are taking one meal daily.
Also alcohol increases the chances of detection of ketones in urine.
