What Is The Window Period For HIV Test?

default
Posted on Wed, 5 Nov 2014
Question: Hello doctor,
I am an 30 year old homosexual adult.
I have been always paranoid of HIV and practice safe sex , but then its never 100% safe.
Last week of XXXXXXX 2014 i was in Bangkok , in a XXXXXXX party i was high on alcohol , and i had deep kiss with 2 guys , and 2 guys had performed oral sex , i received oral sex and ejaculated in to their mouth .we had deep wet kiss. I have bleeding gums at times , but that night i didn't have.
2 months 9 days after that incident i had got tested (rapid test) and report was negative.
But I know there is a 3 months of window period , I want to know how much of risk I am at.
Meanwhile i had safe penetrative sex with 1 guy . When I am erected I leak a lot of precum , does it increase the risk?
the questions:
1) minimum how many days it take to form enough HIV anti body to be detectable in rapid test.
2) oral sex , receiving or giving , how risky it is.
3) in XXXXXXX parties from infected needle prick getting HIV , is that a myth.

And How about Hepatits B?
God forbidden if I am diagnosed HIV + , Will I have tpo start ART rightaway?
I am one severe IBS patient and cannot digest many kind of food ,I am on medication for it. Does ART have lot of side effects?

Thanks & Regards XXXXXXX

Brief Answer:
Answered following questions.

Detailed Answer:
Hello,
1. In the field of Medicine, nothing is absolute and 100%. Evidence shows that at the end of 3 months (90 days), 99.9 % of infected persons will develop antibodies in their blood and
The tests would pick up them. So the barrier of 3 months is to be considered as 'window period'
2. As per CDC (Center of Disease Control , USA) the risk of acquiring HIV through unprotected oral sex is low whether receptive or penetrative and they are considered low risk activities when compared to vaginal or anal sex , (http://www.cdc.gov/hiv/policies/law/risk.html),
Though unprotected receptive oral sex is riskier than penetrative, as the ejaculate of the person performing oral sex will be in contact with possibly infected semen for a longer period of time in the oral cavity of the person receiving the oral sex.
3. The risk of getting HIV through needles (infected) is 0.3%
4. Yes, risk of acquiring Hep B through direct blood contact which is the main routes (blood transfusion/needle stick) is many folds more (when you compare it with HIV).
Hope this helps.

Hello Doctor,

Thanks for the information.
Will you elaborate me little bit about the treatment process of HIV?

"unprotected receptive oral sex is riskier than penetrative" ...who is receiving oral sex ? the person whose mouth is around the genital or otherway

" the ejaculate of the person performing oral sex will be in contact with possibly infected semen for a longer period of time in the oral cavity of the person receiving the oral sex."

Thanks & Regards XXXXXXX

Detailed Answer:
Thanks for your response . Here is the elaboration
The riskier person is the one who receives oral sex (receptive oral sex) , meaning thereby the person in whose mouth ejaculation occurs if it is unprotected (without condom)

If the sex occurs unprotected (without condom ) then the person penetrative oral sex will ejaculate in the mouth of the person receiving the oral sex ( receptive sex).
and the ejaculation will remain for a longer period of time in the mouth of the person in whom it has been ejaculated while the person's male organ ( performing penetrative oral sex ) will remain inside the mouth of the person( upon whom oral sex is performed ) for a very brief period may be one -two minutes.
Naturally he (performing penetrative sex ) will be at a lesser risk.

TREATMENT OF HIV/AIDS :
there are myths that every HIV + person will need anti HIV medicines to start with. Only those HIV + persons will require anti HIV medicines whose immunity status is low which is determined by the level of one 'CD4 cells' in the blood and once it drops below 350 cells/ mm3 then only we can start the anti HIV medicines.
There are bucketful of medicines and medicines can be picked up according to the suitability of the patients ,once the decision to start the medicines is taken.
The medicines are ti be taken for life as the medicines for hypertension/diabetes
are taken. Another aspect of treatment is that it requires 100% adherence
But question of taking of medicines arises only when one is diagnosed HIV + and his/her CD4 cell count has dropped < 350/ mm3 level.
Hope this helps.
