What Is The Significance Of Free Fluid In The Pouch Of Douglas?
Question: What is the significance of free fluid in the pouch of douglas? I had it in my last two ultrasounds and I wasn't in ovulation. Also, all my other gynecological results are fine: all the organs look ok, and I was tested negative on STDs. What can be the cause of the fluid? Can it come from the colon? I have IBS and some abdominal pain recently and I am quite concerned about this fluid.

Free fluid in pouch of XXXXXXX is a very non specific finding and is seen in many physiological and pathological conditions like -
- at the time of ovulation
- in pelvic infection
- early pregnancy
- any inflammation in peritoneum due to some infection in colon or small intestine
So I would like to know the symptoms for which ultrasound was done and report of ultrasound so that I can corelate the finding with them.

The first ultrasound was a follicule count, done because husband and I are TTC. I had diarrhea for days at the time the ultrasound was done, since I have IBS for several years. The second ultrasound was an ultrasound of the abdomen, because I have developed pain under my ribs (mostly on the right). The ultrasound didn't show any problems on the abdominal organs, and the only thing the doctor noted is that I have some fluid in the POD. I also dis a complete blood analysis and the only thing that was shown is that I have lower ferritin (11ng/L). An other symptom I have for several months is a stronger urge to urinate, even though my urine was sterile for bacteria.
All of this is worrying me a lot, because I am scared I might be loosing blood somewhere. The only place that I haven't checked recently is the colon. I am scared it might be cancer.

Detailed Answer:
Listening to the series of symptoms and correlating it with the ultrasound findings the most probable reason seems to be either perihepatitis or a duodenal ulcer.
More common is duodenal ulcer because you have iron deficiency too.
Seeing your age cancer seems unlikely until you have a positive family history.
IBS is usually not associated with free fluid in POD.
So in my opinion visit a gastroenterologist and get yourself evaluated.
Urinary frequency can be due to lesser water intake so try to take a lot of water for 4-5 days and see if there is any improvement in symptoms.
But in case it doesn't help then take vitamin C tablets for two days followed by alkaliser syrup for two days and do this alternately for 10 days and see the improvement.
