Posted on Wed, 3 Feb 2021
Question: Hello, I want to take suggestion w.r.t my mother's problem.
She has problem of Early Cirrohsis as indicated in ELASTOGRAPHY report. Actually when my mother felt little inflammation inside stomach then i went to doctor.
Doctor did series of test as follows.
1) LFT REPORT
2) ULTRASOUND UPPER ABDOMEN.
3) ELASTOGRAPHY
4) ENDOSCOPY
5) AUTO IMMUNE HEPATITIS PROFILE TEST.
during all the above test doctor told ki he is trying to find out the reason of chronic lever problem or may be lever cirrohsis.
Now on the basis of latest test report of AIHP
doctor is suggesting for TRANS-JUGURAL LEVER BIOPSY. now plz suggest what is to be done.
1) Is lever biopsy is eesentially required to go ahead further treatment ?
2) Is it related to any sign of checking lever cancer or any critical condition.
3) On the basis of above test report medicine can be given for betterment of lever or not ??
4) what is the chance of survival of such patient and what may be survival period.
5) what course of action should be followed now so that she can be cured??
Plz suggest suitably plz.

Hello,

Firstly lets sum up the reports-
Her antinuclear antibody is positive which is usually seen in patients of SLE.
ultrasound is suggestive of chronic liver disease with gall bladder thickening.
Her mitochondrial antibody are positive which are usually suggestive of liver cirrhosis or biliary cirrhosis.
Lastly she has duodenitis and gastritis.
1) Yes liver biopsy should be done. The cells of her liver have to be examined under a microscopy to estimate the damage to the liver. If there is any dysplastic changes or not(precursor to cancer). She is a SLE patient and this makes the things even worse. She should be examined thoroughly. Advice by treating doctor is good.
2)Yes it is related to checking if there are any changes in cell related to liver cancer. If yes then in which lobes of liver.
3)Everything is treatable in today world but time of detection of a disease plays a pivotal role. The earlier you find it better are treatment option and prognosis of patients.
4)If detected early almost 90 percent plus. We have liver transplant etc., these days and all of these treatments are highly successful but again as said earlier time plays the pivotal role.
5) Once biopsy reports are out we can decide the further course of action because malignancy treatment is fully different from. Cirrhosis etc., treatment.
As of now my suggestion is don't waste too much time. Go for trans jugular biopsy.

Once we have the reports we can discuss further line if action.
Hope i was helpful.

feel free to follow up.
Regards.

Brief Answer:
Follow up answer.

Detailed Answer:
Hello again my dear patient,
Its not necessary that a malignancy always presents with a mass of cells. There can be dyplastic changes inside few cells which in easy language means a precursor to growing malignancy.
Secondly sir cirrhosis and malignancy are totally different things.
Cirrhosis is a late stage of scarring (fibrosis) of the liver caused by many forms of liver diseases. So as evident cirrhosis just means scarring of liver cells ,As they are scared they can't function well over time. Malignancy (Benign or malignant) is a completely different world where either the cells in a particular zone starts changing their pathology with uncontrolled growth and begins to cross blood vessels causing metastasis in late stages.
Apart from malignancy a liver biopsy helps to rule out the following conditions-
Nonalcoholic fatty liver disease
Primary biliary cirrhosis-Suspected in her
Primary sclerosing cholangitis-Suspected in her
Hemochromatosis
Wilson's disease

1)Medanta the medicity Dr XXXXXXX sood.I think the best Doctor in NCR.
2)Go to Medanta the Medicity. Its the best.
3)Some what around 4k.
4)Sir you have taken the policy for your new born and patient is your mother.I can't get the question. Are you asking that if any thing like that happens in future then is it going to help your baby?
If yes then yes.Stem cells are used to prevent numerous condition and can succesfully treat them.Studies and researches are going on in this are and 10 to 15 years down the line we could expect wonders from stem cell therapy.

Hope i answered your query well.
