Generally PSA level for your age is below 3,5 ng/ml. However this may vary depending of the laboratory (every laboratory has its own norm).
Biopsy is usually done in case of abnormal PSA levels or abnormalities in rectal examination or abnormal ultrasound.
So it is possible that your urologist could find something in the ultrasound or during the rectal examination. Your PSA level is a little bit too high so this would be an indication to biopsy as well.
