What Is The Dosage Of Azithral Syrup For Cold And Cough?

Question: hi my 11 month old daughter is suffering from cold since 3 days. since last night she could not sleep due to nose blocking and I can hear bubbles like sounds in her breathing. Mostly the sound is from nose but I have a feeling there is sound also in her chest when she breathes. She has a chesty cough too and fever also since two days. I am giving her nasal saline drops, steam inhalation and paracetamol when needed. She was prescribed Azithral paediatric syrup 3 months back for a similar condition. Shall I start Azithral syrup now. If so What should be the dose? It is 40mg anhydrous azithromycin in 7.5ml. Please Help. Also She was Prescribed Syrup BROZEET LS 3.5ml TDS. Shall I give this too? Her Weight is 9 kgs

Hello

I can understand your concern.Most common cause of cough/cold in this age is some viral infection or allergy.
The symptoms can be due to upper respiratory tract or lower respiratory tract involvement.
Noisy breathing can be there in both the condition.
In case of lower respiratory involvement there will be associated increased breathing rate or poor feeding.
I won't get worried if there is no breathing difficulty,baby is active and feeding well.
Continue giving paracetamol, nasal saline drop and steam inhalation.
I would suggest you to give Brozeet syrup.It contains levosalbutamol 0.5 mg per 5 ml.
The dose of levosalbutamol is 0.05-0.1 mg/kg/dose.So you can give 5 ml three times a day.
Azithromycin is indicated when infection with atypical organism is suspected.
If there is no involvement of lower respiratory tract,I would suggest you to observe the baby.
In case of respiratory distress,poor feeding or persistent fever get her evaluated by a pediatrician.

Thank You doctor for taking out time to answer my question. I would like to tell you that I can hear wheezing in her chest and can see her breathing is shallow and chest movement is rapid. She is crying all the time, only soothed by carrying. She is taking milk but not eating soilds.I am not able to visit a doctor presently.

"Azithromycin is indicated when infection with atypical organism is suspected." Could you please advice How to know if infection with atypical organism is there. Because I am convinced it is infection in the lower tract as fever persists and the symptoms have been there for more than three days. Also kindly tell the dose of Azithromycin for paediatric use and contraindications if any.

kindly advise about the warning signs to look for and when to rush to a doctor.

In this age the most common cause of respiratory infection is viral.The common bacterial respiratory infection in this age is streptococcal or staphylococcal(gram positive organism),not atypical bacteria.
So once lower respiratory tract infection is suspected it is always better to give amoxycillin clavulinic acid combination rather than azithromycin.
Atypical bacterial infection is more common in preschool and school going age group.
In atypical bacterial infection fever is not prominent.

In your query you have mentioned that your doctor had advised her azithromycin for similar complaint and her symptoms improved.
Even azithromycin can be given for gram positive infection but not as first choice.
The dose of azithromycin is 10 mg/kg/day single daily dose.
It is contraindicated if there is known hypersensitivity.
Persistence of fever,irritability,poor feeding,increased respiratory rate all these are danger signs.
So I suggest you to get her evaluated by a pediatrician to assess the severity of condition.
