The difference in both these combination is marginally higher dose of acetaminophen.
The second combination of Pain Killer contains Tramadol 37.5 mg and Acetaminophen 325 mg is the most common combination used. I am not sure if the first combination - Tramadol 37.5 and Acetaminophen higher dose is commonly available. And case you have got it, I would not recommend it.
