Question: hello Doctor

i am suffering from hyperdigmentation from last 3 years,i have treated well with ayurvedic,homeopathy and alipethy as well but nothing work and from last 1 months its darker and darker and spread on my cheeks as well as nose earlier it was jst a dot onface now its spread on cheeks and whole nose

I am having 10 to 12 glass of a water in a day ,ahving juice and fruits as well

i am very worried about my skin,i need clean and clear sking,plz help

I have gone through your query and I have understood it.

You seem to have a condition known as Melasma.
Melasma commonly presents as brownish to dark pigmentation on face, particularly the malar prominences, dorsum of nose. Other areas like forehead, upper lip and chin may also be involved.
There are various treatment options available to fade away Melasma, however, Melasma may recur after seemingly complete cure, if patient discontinues treatment Or is taken off all treatment.
Therefore maintenance of improvement is absolutely necessary after significant improvement has been achieved with treatment.

Sunscreen is an essential part in the management of Melasma.
A sunscreen should be broad spectrum i.e have both UV B as well as UV A coverage.
A sunscreen with minimum spf 30 or more is considered to be broad spectrum.
It should be used in adequate quantity (3 ml for face and neck) and reapplied every 2-3 hours throughout the day, specially if outdoors.
You may find one such sunscreen OTC. Neutrogena dry touch ultrasheer sunscreen is an OTC product with an spf of 50+.
You may also use a sunscreen prescribed by your dermatologist based.

Besides, sun protection, pigment lightening creams are part of the treatment regime.
Active ingredients which are usually present in these pigment lightening creams are hydroquinone, tretinoin, kojic acid, glycolic acid, arbutin, mometasone furoate, fluocinolone acetonide etc.
These ingredients are either present alone or in various combination.

The mechanism of action of these active ingredients is either exfoliation of existing pigment Or inhibition of synthesis of new pigment Or both.

Usually a synergistic combination of ingredients is used to maximize the results e.g in a combination of tretinoin, hydroquinone and mometasone.
This combination is a gold standard for treatment of Melasma against which other treatments are compared.
In this combination hydroquinone and mometasone inhibit melanin production whereas tretinoin exfoliates existing pigment as well as enhances the penetration of other active ingredients i.e hydroquinone.

Along with topical treatment, you may also opt for a few sessions of chemical peeling from a dermatologist.
Glycolic acid peel is particularly useful for melasma.

Once significant improvement has been achieved in melasma, it is necessary to maintain improvement with regular sunscreen use and a maintenance pigment lightening cream e.g active ingredients like agents like kojic acid, glycolic acid are ideal for maintenance.
