What Is The Cause And Treatment For Infertility?

Posted on Sat, 28 Feb 2015
Question: Dear Sir
We have been married since 7 years and have been trying for a child since. As on date, I am 32 years old and my husband is 38. I had first underwent a DNC for polyp removal in March 2009 as I had symptoms of abnormal bleeding. I was weighing 100 kg at that point of time and at present I weigh 96 kg. Between January 2012 and XXXXXXX 2012 I underwent infertility treatment wherein doctors examined me for follicular study and growth. Thyroid at this stage was within range but slightly higher. Doctor prescribed me thyronorm 25 mcg which I am taking till date. Doctors also asked me to take fostin 75 injections (between day 7 to day 11) during the period March 2012 to XXXXXXX 2012. In XXXXXXX 2012 I had a chemical pregnancy where the results were weak positive. At that time I had slightly high cholesterol levels. Further, in December 2012 I was diagnosed with PCOD and a polyp was also observed for which in January 2013 I underwent laproscopic surgery specifically for PCOD treatment(ovarian drilling) and removal of polyp . This surgery consisted of diagnostic laparoscopy, hysteroscopy with polypectomy , dilatation and curettage. In May 2013 I conceived (LMP 4April 2013) and the scans showed that foetus (6 weeks 3 days) had heartbeat but was high (184 bpm) and could be appreciated only at intervals. I was also taking HCG injections at this point of time as it was not rising. In XXXXXXX 2013 I had a complete abortion. Due to this the doctors checked my vitamin D which was 11.61 ng/ml in September 2013 and I was asked to take vitamin D injection and continue with tablets. Between September 2013 and XXXXXXX 2014 my weight increased to 112 kg. Since XXXXXXX 2014 I went on a weight loss diet and reduced 16 kg to around 96 kg in November 2014. My LMP was on 25 November 2014 and I conceived in the month of December. Foetus with hearbeat was discovered on 16th January 2015 and heart rate was normal ie 140 bpm and everything was normal. I was overjoyed since after lot of struggle I had conceived and my next scan was on 16th February, 2015 . But to my horror on 2nd February, 2015 I had a bit of brown discharge and in a state of panic I contacted my gynac . On examining me she said that I had a missed abortion (foetus 6 weeks and 2 days) . The foetus CRL was 8mm and gestation sac was 16 mm. On 3rd February. 2015 I underwent a D&C for removal of foetus.
This put me into a state of acute depression and sadness. Doctor is now advising further tests since I am already 32 . These tests areHBA1C, FBS Fasting S Insulin, Antithrombin III levels, S homocysteine levels, Protein C and Protein S activity, Anticardiolipin antibodies IGG, IGM (B2Glycoprotein), Lupus anticoagulant, karyotyping and Antids DNA etc and for my husband a semen analysis test and karyotyping. All these tests are to be done after 3 months . Though the doctor does not find anything visibly wrong she wants to investigate.
On our part we are extremely eager to have a child and cannot afford to wait for 3 months . Request if some medicines which can prevent miscarriage is informed to us. We also want to know if these tests are absolutely necessary since they are on the expensive side. Further, even if they are necessary why should they be done only after 3 months.

Brief Answer:
different tests advised

Detailed Answer:
Hello fear, I understand your anxiety and all the panic you are going through.
These tests are definitely very expensive but at present stage you need not to get all of them.
I want to mention only a few very important tests and just get them done and then we can discuss further management accordingly.
No need to wait for another 3 months for conception.
Get the following tests -
- fasting and post prandial blood sugar levels
- antiphospholipid antibody
- anti cardiolipin antibody
- TORCH IgM antibody complex.
Just get above tests and upload the results.
All the best.

Hi Doctor,
I have uploaded the reports of the tests you suggested.
Also, I am having light bleeding during night similar to spotting with brown discharge. Its generally on and off.
I would like to know generally when should I expect my period. Mine was a 29 day cycle before my conception.
Even assuming the other tests are to be done, are they to be done after a gap of 3 months or can be done anytime or after I get my first period.
Awaiting your reply.

Brief Answer:
irregular cycles are common after abortion

Detailed Answer:
Hello mam, all your tests are normal and thus good to see the reports.
It is common to have irregular periods after abortion.
You might get your periods on time like on 3 march or so but can be sooner than that.
The tests can be done anytime but better if you get it after 6-8 weeks after abortion.
