1. The testing method applied was not sensitive to your infection as details of your testing method is not known to me and not every test is completely accurate.
2. The antibiotic you took might have killed the chlamydia. Even single dose of a powerful antibiotic is sufficient to cure the condition.
If you take complete treatment, it will be cured in most probability. Please do not worry.

I have seen through your report. It mentions optical immunoassay method of testing and there is a little possibility that the test result might be false negative. Ampicillin is not usually a medication given to treat chlamydia.
I can see that the urine culture done at that time shows no growth. I further suggest that you please get a repeat urine culture and sensitivity test done and also culture of the discharge if there is any. This will help in complete evaluation.
Also drink lots of water and fresh juices and visit washrooms regularly.

Chlamydia trachomatis (25 to 60 per cent).
Mycoplasma genitalium (up to 25 per cent).
Ureaplasma urealyticum (15 to 25 per cent).
Trichomonas vaginalis (17 per cent).
Herpes simplex (rarely).
It is clear that Herpes is the last possibility. Usually herpes appears with burning, itching and development of blisters in two weeks. Rarely can it cause a problem like the one you have.
