Posted on Tue, 30 Dec 2014
Question: For Dr. Rovena Murati.

thanks for your replay . problem is ,its not possible for me to test because i came out from anxiety very much . so if i again start thinking of test ,i will be again in same phase . these are my physical conditions that gives me these thoughts again and again . i ask you questions because whenever i relate my thoughts to hiv i feel more anxiety. i also know that my chance of getting hiv was very less as i have studied on internet . i can do any test other then hiv. and you are right i am using multivitamin tablets but with half quantity as i was taking before. . i have less feeling of under chin glands. means i already told you i have some sore throat type feeling thats also very less now. but this muscle vibration and leg vibration was on and off from some days.
the problems is all of these symptoms are on and off , thats give me more anxiety.
means i feel muscle pain not permanent, some time just for very less time.
and also these stomach problem i never felt like this before. these conditions gives me thoughts.

Hello XXXX

I apologize for the delay.

In my opinion (considering how the incident happen) you don't need to get tested since the risk is very low.I suggested it because it seems you can't let go of this idea and it causes you anxiety. So if you run this simple test, you can be sure you are not infected and this may decrease your anxiety.
You don't need to be anxious about the test so I would suggest to do it so you can put a end on it.

Considering all your complains and symptoms ,they progress during all this time, I can tell that they are not related to a specific serious disease.
They are triggered from anxiety.
It is typical for Anxiety disorder to cause such symptoms that come and go .
So probably now would be a good idea to think about treating your anxiety .
Since you can't find any therapist in the place you are now,you should consult a specialist when you go home (preferable a psychiatrist or a general physician).

At this point is important to treat the real cause of your symptoms (anxiety) or you will continue to experience such symptoms.
The doctor after examination will decide if you need treatment for your anxiety or you can manage it with supportive help (therapist).

Let me know for any question you may have.

Dr.Rovena

yes i am agree with you. if i think just for some time without anxiety i can understand that i have very less chance of getting infection.
you can understand how much fear hiv can create. but still i was out of anxiety very much from last month but this week again i got that.
now from some days i feel a little pain in groin area. its not a permanent pain. and when i feel ok then even by pressing that area i will not experience pain. but sometimes i feel slight pain. what could be the possible reason? sometimes i think it is lower abdominal pain. but i cannot find the exact place.
and today i feel in my lower chest or ribs left side pain . and this finished at night now. i dont knw why such type per pain comes.

Brief Answer:
it doesn't seem of concern,

Detailed Answer:
Hello again!

Well groin pain is most of the causes is caused from a muscle strain.Other less common causes may include inguinal hernia, kidney stones, joint inflammation etc.

In your case since the pain is mild, not permanent ,that doesn't increase when you press on that area, you don't have any other symptom, is very unlikely that it is caused from a specific condition.
Since you feel it in the lower abdomen ,most likely it is due to abdominal cramps or cramps of abdominal wall muscles.
The fact that it spreads in your lower chest or ribs ,make it highly likely to be a muscular pain.
Everyone can experience such symptoms time after time. The only difference is that the majority of them don't pay attention and you because of your anxiety are more alert and notice every small changes in your body.

I can tell for sure that you don't need to worry about this.
I know it seems difficult right now but try to relax and to ignore these kind of symptoms.

Regards,
Dr.Rovena

Hello again XXXX

Apologize for the delay.

A pain that is slight and changes location constantly ,is very indicative for muscle pain or for anxiety induced pain.It is not related to a specific disease.
It doesn't seem to be related to stomach.

It is more common to experience this kind of body aches in winter because due to low temperatures and the lower barometric air pressure.
You don't need to worry ,there is no specific age for muscle pains ,everyone can have such pains.Important is that they are not related to a disease.

Try to stay warm and wear warm clothes during this period.

Take care,

Detailed Answer:
Hello XXXXX

I saw the picture you send.I notice one pimple near nose.It is a common pimple and every person in this planet has them time to time.
There are 4 factors that contribute in pimples formation :
1-Excess sebum production by sebaceous glands;
2-Outlet obstruction of the sebaceous follicle arising from excess production of keratinocytes( occlusion of the pores).
3-Increased proliferation of the bacteria Propionibacterium acnes that normally live in the sebaceous follicle.
4-Inflammation caused by sebum escaping into the surrounding skin.
The areas most commonly involved in acne are the face, upper chest, and back.

You really don't need to worry about this. You don't seem to have any problem with pimples. It is normal to have any of them once in a while .
The pimple near your nose is not inflamed .It is just a small pimple.

The fact your pain changes location constantly indicates a muscle pain or a pain induced from anxiety. For this reason when you have pain in one location ,it disappears from previous location because your attention is placed in the new location ignoring the rest. This is typical for anxiety induced symptoms (or psychosomatic symptoms).
Usually swollen lymph nodes are painless and don't cause any kind of discomfort like burning feeling. They are discovered only when they are touched accidentally.
In other cases they can be really painful to touch and swollen. In this case the cause is inflammation due to an infection in the body.
You don't have any reason to think that you will have swollen lymph nodes.
Try to relax and not to think any more about this.

Take care,
Dr.Rovena

and my lower body vibration that i have discussed many times how that still i have . its not permanent it sometimes i could not observe one or 2 days. and sometimes i observe in evening . this is the most troubling symptoms that i have.

Hello again!

Anxiety doesn't include only panic attacks or thinking all day that you have a serious disease.
Anxiety comes in many different forms.
Anxiety may start in the mind, but it often manifests itself in the body through physical symptoms, like chronic digestive problems, muscular pain, different sensation in the body.
Since you are always worried and even minor ,insignificant symptoms make you think that you have a disease, this means that you have anxiety.
You may not think all day about cancer or HIV but are always observing for new symptoms on your body and in this way noticing even minor changes that aren't related to any disease.
If your symptoms were caused from a disease ,they would be persistent and progressive in time. The fact that your symptoms are slight and are on and off, it is unlikely to be due to a specific disease.

Regarding the vibration sensation in your lower body there are 2 possibilities:
1-It is due to anxiety and it will go away once you can control your anxiety or get it treated.
2-It is caused from a disease:In this case there are some possibilities including:
-Electrolyte imbalance :-You should test the level of Calcium,Kalium ,Magnesium in blood
-It is due to Vitamin B12 and folate deficiency :In this case you should test Vit B12 and folate level in blood and run a complete blood count.
-It is due to an neurological disease :In this case you should consult a Neurologist for neurological examination.

Since this sensation is not persistent ,is not progressive ,it is less likely to be related to neurological disease, but if this continues you can consult a Neurologist for examination. So you can put your mind at rest and get over this.

Hope I have answered your queries.
Let me know if you need further help.

Take care,
Dr.Rovena

Dr thanks for replay..
you have written in 2nd point that if it is due to some disease. so what could be the possible disease? that can cause such vibration. and what does persistent means. when i have such feeling i can observe for many time but its not for all day. i can feel actual trembling or vibration.

Brief Answer:

Detailed Answer:
Hello again!

Well persistent mean when you are having the same symptoms at the same location for a while, every day and you can feel it most of the time during the day.

If this vibration sensation is persistent it could suggest nerve pathology.
If the area of vibration is localized and does not change, it may suggest that the sensory nerves supplying that area are getting affected by either compression of the nerve probably from a protruding intervertebral disc,Diabetes or other causes (Vitamin deficiency ).

Sometimes a sensation vibration in legs has been related with keeping a phone in pants pocket or on belt.

In your case it doesn't seem to be related to a disease. It looks more like a muscle twitch but since this is causing you anxiety ,you should consult a neurologist and get examined to exclude these others possible causes.
