What Is An Effective Test For HIV?

Thanks for your reply.
As what i said i did the Elisa combo test after 23 days, and the result is negative.
The case is i had unprotected oral sex on 3rd of XXXXXXX And i took the combo test. Is the test reliable ?
And made me very stressful is there are some symptoms appear these few days like mild sore throat and swollen gland not feeling well, white tongue, and low fever appear for one night.
And painful feet.
It drive me crazy. Dr pls give me some advice!

Welcome and thanks for posting your query to the forum.
I can understand your concern for HIV after unprotected intercourse.
Let me first reassure you that your symptoms are not at all matching with typical Antiretroviral syndrome or also known as ARS of HIV.
In ARS there is high grade fever with typical rash and frankly enlarged lymph nodes. The typical of symptoms are like patient says "worst flu ever".
So getting to your symptoms I can not find high grade fever, no rash and not severe of sore throat.
For lymph gland enlargement - I very often see young men like you coming and complaining of node enlargement but on examination I find that they perceive the normal structure of muscles and other normal tissues as nodes.
White tongue is generally a sign of dehydration and stress.
So getting to all the points your symptoms are not typical of ARS and rather are part of anxiety and stress due continuous thinking and reading about of HIV.
Regarding your combo test- Before I guide you for the test let me first guide you about the standard protocol which we follow and about HIV infection:
In an unprotected intercourse with a HIV positive person the HIV viruses gets entered into the body and divides in innumerous numbers(millions of viruses per ml).
After a period of 2-6 week there is mounting of antibodies against the viruses. These antibodies can take at the maximum of 8-10 weeks of the period to get expressed.
Therefore the infection can be analyzed by two ways -
-A): By HIV virus detection: done by RNA- PCR. It tells us the number of copies of virus present in per ml of blood. By the latest of methods the viruses can be detected within 2 weeks of period.
B): By HIV antibody Detection: As told earlier once the antibodies can appear from 4th week to maximum of 8-10 weeks and then these antibodies to HIV virus persist throughout life.
The protocol of the antibody tests done for HIV is as follows: One HIV antibody test at 6-8 weeks and then a follow up final test at 3 months (84 days or 12 weeks).
Antibodies once formed to HIV can never disappear. So if a person has HIV infection, 3 months of period is the time when we say to be fully sure of whether HIV is present or not on the basis of antibody tests.
Now correlating all the facts and interpreting with your situation:
- It is good that your ELISA combo at 23 day is negative but we take this test most conclusive at 84th day.
- A RNA PCR test is of best help in all the anxious cases who do not want to wait till even till 6 weeks -12 weeks.
So to relieve all the anxiety I suggest you to go for RNA PCR.
Rest I again reassure you that your symptoms are not at all matching with HIV illness and therefore do not worry about the same.

I really feel bad and always checking the symptoms on my body, like this evening i feel my armpit swollen lymph node after i drink a glass of water.

I had few times unprotected oral sex on 20/5, 28/5 and the last time was on 3/6. And i did an instant blood test and had a negative result, then i did a fourth generation elisa combo test on 26/6 and a negative result received on 27.
I already not sleep well for 2 months, just around few hours a day.
Since last week, there were some symptoms show up in order. First my swollen gland not feeling well, then mouth ucler, low fever for one night. Then my legs not feeling well and feet are painful. Then i not my armpit not feeling well, and there are few cold sores appear these few day around my lips. And mild diarrhea this morning. And i felt my body water expansion today.I feel very stressful. is the combo test negative is wrong? I did a RNA test yesterday and result will be on coming thursday.
Please give me some advises , am i infected?

Let you understand each and every point in detail-
- In ARS three is swelling of multiple nodes and not a single node.
- There is fever of high grade remaining for atleast minimum of 7 days to up to 14 days. Your fever remained only for one day and that too of low grade which has no significance in HIV.
- Mouth ulcers are not XXXXXXX manifestation of HIV and anybody can have oral ulcers due to stress.
- Restless legs are again due to stress and anxiety and are not symptoms of HIV proper.
- Cold sores is very common to occur due to stress so do not correlate it with HIV.
- Diarrhea is a broad term and we should have at least passage of 4 or more stool of lose consistency to designate diarrhea. SO do not think only of HIV.
As I said based on all these facts as well as type of exposure you have is not giving any indication of HIV. That is why I am cent percent confident that your test of RNA PCR will come negative. I suggested you RNA PCR only for the part of giving you a medical proof in your hand therefore the confidence that you are clear.

I m still very nervous bcoz when u mentioning aboout ARS. I got sore throat since last thursady and swollen gland not feeling well after. Then i found my tongue quite white in color in the inner part, then i felt very tired.... Joint paiunful..... And yesterday start not feeling well with my armpit on monday. The start diarrhea yeaterday. But just very low fever on last wedneaday night. I know i m very terrify....by then what is "Infectious mononucleosis"?
And will be the symptom skin rash next?......

To tell you more importantly as per the data most people will get infected with EBV in their lifetime and will not have any symptoms. We ourself are not able to remember when we can get infected with this virus.
So you can see that it is not at all uncommon but one of the commonest viral infections seen in world. I don't know why you have gone for the report, probably after reading on internet regarding Infectious mononucleosis and thus to rule out the possibility of the same.
But lucky man, your reports do not show any recent infection of EBV and thus for infectious mononucleosis. If you can see the report then can interpret that none of IgM antibody is positive and only IgG antibodies are positive.
IgM antibodies are positive in recent infection, say for within a period of 2-3 months of exposure. IgG antibodies are positive in case of past infection of many months to years to indefinite period. All of might get infected with EBV in childhood and can show positive IgG antibodies. You can see that both Anti aVCA and Anti EBNA IgG are positive. The presenace of antibodies to both VCA and EBNA suggests past infection (from several months to years earlier).
This EBV is so simple virus that it can get spread by sharing drinks and food
using the same cups, eating utensils, or toothbrushes or simple kissing and toys etc.

Dear Dr. XXXXXXX if about the ARS high grade fever, when will it normally happen after the esposure. I just suddenly alert about the HIV infected about 2 weeks ago. From 4th XXXXXXX to 2x XXXXXXX maybe i really not feeling myself fever?
My last unprotected oral sex was om 3rd of XXXXXXX So would it be already happened and i never noticed? By then the 24days elisa combo test was done on 27 june? What is the possiblity the antibodies not show up yet?

The rash is simple thing and not related to any viral agent. I am not able to get RSA. Kindly elaborate about RSA and give me full form.
Regarding restesting: RNA PCR is the only test which detects virus itself and there is no other test which can detect directly the virus particles. If you ask for authenticity of the test and comparing it with any other test then it is like showing torch to sun.
I think you can get from this thing that how much reliable is this test.
In case of going to any other lab- you know from wherever you have gone for the test, it is the credibility of lab which is at stake. Especially for HIV the things are very much sensitive. I have never seen in my clinical practice or heard anywhere about in-authenticity of RNA PCR and wrong reporting.
Even if you go for any test whether PCR or combo it will be negative.
