The commonest cause of red and painful perineum in men is contact dermatitis. Other causes may include superficial bacterial infections, eczema, STDs like Herpes, crohn's disease, etc.
In most of the cases it shows improvement on taking measures like-
* Avoid scrubbing your skin.
* Use as little soap as possible. Use gentle cleansers instead.
* Use warm (not hot) water for cleaning. XXXXXXX dry, don't rub.
* Try calamine medicated lotion.
* Hydrocortisone cream (1%) is available without a prescription and may soothe many rashes. You can use it along with ointment neosporin twice daily.
In case the symptoms are persistent then a direct examination by dermatologist will be helpful in identifying the cause and specific treatment.
