Question: Hi

My sister is in first trimester
Attached is the her Thyroid results

Please suggest what they mean and what should be done now
I can see 2 results are not normal

I can understand your concerns for your sister.
I have gone through the thyroid reports. TSH level is low and T4 levels are higher than normal.

My first suggestion for her would be to go for Free T4 and Free T3 levels.

I would like to tell you some facts about thyroid levels in pregnancy. This will help you understand the scenario better.

After conception, total T4 concentrations increase by 6–8 weeks and may remain high till delivery. Thyroid-like activity of hCG (hormone secreted during first trimester) results in a decrease in serum TSH in the first trimester. Therefore, during pregnancy, women have lower serum TSH concentrations than before pregnancy (Gestational Hyperthyroidism).

Kindly tell whether your sister has following symptoms:
failure to gain weight,
heat intolerance,
excessive sweating,
and increased heart rate, beyond that normally associated with pregnancy.
Any History of thyroid disorder before pregnancy?

Presence or absence of these symptoms alongwith Free T4 and Free T3 levels will guide in proper diagnosis and determine the line of management.
