Question: Hello, The query is for my Grandmother who is 79 yrs old. Please find attached her TSH profile report. Can you please review the reports & share your view?

Please let me know if you need any additional info.

Dear XXXXXXX Hi! welcome to HCM. I am dr. Minal.
I have gone through the reports of your grand mother. She is a case of primary hypothyroidism. As of july'16 report TSH was 32, probably you have started the treatment there after, which you have not mentioned, next report as of august '16, is TSH 14.
She is 79 years, i would like to know her weight for dose adjustment.
sugars and the remaining report seem normal.
Is she on any treatment? like for diabetes, blood pressure, cholesterol, thyroid or anything else. do let me know.

Jul-16 report talks about TSH. As TSH was high, it was suggstd to do FT3, FT4 & Anti TPO test. The Aug-16 report says TPO is 14. I got doubt from your above reply, so just confirming if TSh & TPO is same.

But anyway, we haven't started any medication for her yet. And she was not on any sort of medication in the past and at present also.

Her weight is 46Kg. Hope this detail helps you for further advise.

TSH needs to be treated.

Detailed Answer:
Dear XXXXXXX I apologize for my mistake and a hurried look. Usually FT3, FT4 are reported along with TSH so I took it for granted that the value is for TSH . Sorry again.

Now as of your query, TSH is high, with low FT4 and normal FT3, TPO is normal or negative as we say. this is still a case of primary hypothyroidism. TPO negative means that it is not autoimmune in nature but senile i.e. age related.
Her weight is 46 kg, and age 79y, so let us start with 25 mcg of thyroxine once a day, empty stomach, early morning, and observe another 30 min fasting over the tab.

For how many days, this tablet needs to be given?

Can you please give a little more detail, that what you mean by 'Observe another 3omin fasting over the tab'.

1. the tablet needs to be continued for long. TSH and FT4 need rechecking after 3 months.
2. the tablet has to be taken empty stomach , early morning, and she must not eat or drink anything over the tablet , except water, for another 30 minutes.
