There are changes in the exercise ECG > “Positive for inducible Ischemia” - suggesting that a part of the heart may not be getting enough blood during exercise
This may mean
A false positive test – no disease, but changes in ECG
Or Coronary Artery Disease, for which you need further investigations … The aim of any investigation is to modify the treatment, based on the result.

     It is an indication for further work up –.
     The next step is TMT with thallium isotope. It is the ideal non-invasive way to evaluate ischemia / to assess the PHYSIOLOGY (function) – to see whether the blood arriving at the heart muscle.
     And we should see the ANATOMY (structure) – CT angio is non-invasive study for the anatomy. If positive, she will need catheterisation – coronary angiography with a view for possible intervention. It is the way to directly ‘see’ the block, if any – and its location, extent, severity and so on. Coronary arteriography is invasive but it is the gold standard for this.

Absence of symptom is no proof of absence of disease
Investigation is the way to exclude

Thanks Dr I had a history of Angioplasty implanted 2 drug eluted stents in XXXXXXX 2014 post that I am leaving healthy life with regular exercise regime. can run upto 10km without much efforts. this is quite surprising. taking medicines Prax, ecosprin, metxl and consivas daily. only complication 3 months back I started external application of regain 5%lotion on my scalp for hear growth treatment which has some complications for heart beat abnormality last week I stopped using it feeling quite better..don't want ro go for me or treatment my ECG report is ok. kindly advice

The stents are treating the effects, not the cause (which is often inherited).
You are on appropriate treatment…
The new medicine cannot account for ECG changes…
Still, there could be progression of the disease – in the native arteries or narrowing of the stent itself

The wise course of action / only scientific option – in such a situation is to repeat the angiography, as soon as possible. Avoiding is risky – at least go for CT angio and thallium stress ECG
have a second opinion

thanks Dr as indicated and informed by dr performing stress test the postive sign is very minor what would you suggest on the test need to be perorm as u can understand angiography can be performed if it is high risk ...i had lack of sleep and food habits in past 15 days could be a reason of result in stress test? kindly help invasive way to treat otherwise I am feeling quite ok and energetic unlike last time when I had been through similar situation in 2014

With the present technology, the risk of angiography is practically (though not theoretically) nil or negligible…
Stress can certainly cause ischemia and changes … As a matter of fact, stress (Type A personality) is one of the risk factors for heart attack.
We cannot attribute the changes to stress and leave it – whatever may be the underlying issue (stress or not), this result needs definite investigation and intervention

Symptoms rule in a diagnosis / absence of symptoms do not rule out.
In a quarter of patients, sudden death or heart attack is the first symptom. When there are treatment options available … It is wiser to look and see rather than wait and guess… better be safe than sorry
