What Does This Semen Analysis Report Indicate?

Question: I had me SA done 4 months back. The sample was given to the clinic after 25-30 minutes (From home). It was observed to be above 2.8 ml. I received the following observations from the center:-
a) Concentration - 44 mil/ml
b) Total Count - 123.2mil/ml
c) Motility - 44% --> Progressive - 35%; Non-Progressive -9%; Dead - 56%;
d) Ph level - Alkaline
e) Normal Morphology - 1% --> Abnormal cells - 99%; Head defects -64%; Neck and midpiece defects - 20%; tail defects - 14%
As my SA analysis fine and normal,my Question are as follows:-
We are trying to conceive since 3 years. But every one is blaming my morphology for the problem of conception.
Can my morphology be changed? Are there any medications or injections for this? Please help me out for this matter. As I feel there are no further defects in my body.

I can understand your concern.

Your total sperm count is normal. But motily is low and sperm are abnormal in morphology. Your most of the sperms are defective and that can be the reason of your infertility.

Morphology and motility can be changed by drugs.
Drugs like clomiphene citrate is useful to improve motility and morphology. It is prescription based medicine so consult your doctor and take treatment accordingly.

Can you please clarify these doubts that I have:-
a) I had done the same kind of SA in Feb 2015. It had given a normal morphology of 64%. Even my motility was higher (nearly 70mil/ml --> progressive at 55%). Doctors usually say that morphology cannot change. I have not taken any drugs as such since then nor had any incidents affecting my body. Will morphology suddenly decrease?
b) Will morphology reports from each lab differ?
c) If there is a treatment, how long will be these kinds of treatments?

There may be some problems in your semen analysis report.

Morphology can decrease but this kind of large variation is not seen commonly.

Lab to lab variation is very common because it is done by manual observation and subjective error is common. But variation of 64% normal to 1% normal is not justified by laboratory variation. Laboratory wise variation is 10% to 15% only.

So in my opinion you should go for one more semen analysis in good lab. That will give you exact idea. Then plan accordingly. Duration of treatment depends on many factors like count, morphology, FSH level and other.

So go for semen analysis and give me follow up with your report.

Thanks for clearing all my doubts doctor. You have made me feel more relieved. I have just one small question on these reports. The Feb 2015 report was done in SRL diagnostics (the one with more morphology) and recent (less morphology) in an infertility lab. Which one will you trust as a doctor.
