Question: Hi Doctor,
The Neck Ultrasound Report says :-
"Multiple small reactive Lymph nodes along the IJV and in the JD region on both sides"
What is the meaning? Is it a worrysome? What is the treatment?

Any soreness of the lymphnodes felt by you or by your doctor? and for which the USG was done.
Whether any FNAC Biopsy of the neck lymph node was done in the past?
Did you get recurrent Tonsillitis attacks/with flue complaints frequently?
Update any Complete Blood test / ESR reports done lately.

Do you have any illness in the past/ update any medicines taken for it?
Update if Any body in your family suffered from long cough/ TB chest/Diabetes etc?

PFB the answers:

why this USG was done and for which complaints of yours? - BECAUSE SHE FEELS SOME SORT OF PINCHING SENSATION IN HER THROAT. THIS IS GOING ON FOR A WHILE NOW (NEARLY 6 MONTHS). HOWEVER, IT IS VERY IRREGULAR. (EG: FOR A WEEK THERE IS NO PINCHING/ITCHING SENSATION IN THROAT AND SOMETIMES THERE IS A LOT OF SENSATION FOR A DAY OR TWO)
Besides this I would need much more information to reply your queries and to plan suggestions to treat them,as asked by you-

Did you have any low grade fever/weight loss / with throat pains? - NO GRADE FEVER, NO WEIGHT LOSS, ONLY PINCHING SENSATION IN THROAT

Hi,thanks for your last updates.
I reviewed the updates and studied them in context of your query in relation to your USG report and lab reports.

Based on the facts and the anemic blood status-

*Investigations suggested before Treatment-
I would suggest you to undergo following checks under guidance of your physician and your ENT Surgeon-
-Chest X-ray
-Mountaux test
-Immune Gamma Release Assay(IGRA) -Blood Test-for detecting the latent or the old / chronic evidence of TB
-Throat Swab -for bacteriological studies and AFB study
-AFB of the Morning Sputum,as you have 6 mths history of Throat itching
-Stool for occult, and amoeba/giardia/ ova cyst of worms
-FNAC Biopsy supported with Excision Node Biopsy if need be- would fix by Histopath study confirmation of the cause of the Reactive Adenitis.
Till this time you need not worry ,as the Reactive Adenitis are of academic importance and have very less clinical diagnosis.
Mostly they are caused from unknown viral / or TB acid fast bacteria with sublinical impact.
But as you have ONLY as the effect of your complaints in the throat,It needs to be investigated before the following suggested treatment.

All these tests need to evaluated in relation to anemia you have.

*Suggested Treatment-
You need to contact your treating family doctor for the script and get these treatments under his cover-ONLY.
*Symptomatic Treatment suggested-
At this stage ONLY symptomatic treatment supplemented
with anti-anemic (Iron fumerate+Folic acid+Vit c)
and Multivitamine (Zicovit)drugs would suffice
-Tab Vikoryl- L-for throat itching to control the attacks only
-Sos-Antibiotics-to control secondary infections with seasonal changes.
*Life style and Diet-changes suggested-
-Throat Saline Gargles +Listerin antiseptic gargles initially would help you before taking any of the drugs for exacerbations of infected throat,if need be.
-Good healthy,high protein,bland non spicy diet,
-8 hrs sleep
-Banana Milk shake x 2 times per day
-Sweet Butter Milk x 2 times a day
-Pineapple and lemon juice as desired.
-Tomato juice as needed.
-Spinach with black paper Soup-2 times a day,
would be helping you to recoup your immune status fast.

*Follow up with your doctors-
and continuous care of your throat in time to come is suggested

-What is the meaning? -Reactive adenitis is a diagnosis which is not much of Clinical importance as it is without much symptoms.
-Is it a worrysome? Its nothting to be worried unless further FNAC and other tests confirms the clinical effects of it as suggested above.
-What is the treatment?Treatment-Supportive Symptomatic / Diet therapy / and treatment for the cause after proving the diagnosis as suggested above.
