Brief Answer:
Drastic life style modification to be followed for 3 months!

Detailed Answer:
Hi
Firstly thanks for choosing health care magic to ask your query!

A low carhobhydrate diet is not mandatory for you ,your blood sugar and Hba1c is perfectly normal.
You should take a diet which consists of-

60% carbohydrates - yes carbohydrates are essential for body, but you should make sure to take carbohydrates with low glycemic index .
Glycemic index means that how fast the carbohydrate coverts into simpler sugars like glucose when taken, food with high glycemic index converts into sugars quickly, hence increasing the load on pancreas to secrete insulin, Therefore a diet containing 60% low glycemic index carbohydrates should be taken .

Low glycemic index carbohydrates involve wheat ,barley ,oats avoid rice .
Include lot of vegetables in your diet ,try to avoid red meat, yolk of egg, dairy products like cheese ,butter ,cream.Eat cooked food one time and if possible try raw vegans diet the second time.

Protein should constitute 30% of your diet- take egg white ,skimmed milk ,chicken breast in food, pulses ,beans, nuts like almond and walnut avoid cashewnuts .

Fat should constitute 10% of your diet ,try to avoid mono unsaturated fatty acids like butter ,peanut oil ,ghee instead take oils like olive oil ,rice XXXXXXX oil in your food.use two types of oil in alternation with each other every month for example if you use olive oil this month then use rice XXXXXXX oil next month.
Atleast 30mg of fibres are to be taken every day ,include a lot of vegetable salad and fruits in your diet .
A low salt diet should be followed ,sodium should be restricted to 2grams per day.
Dring plenty of water and keep your body hydrated.
Smoking and alcohol is strictly prohibited

Now lets come to your lipid reports
you triglyceride level is high and your hdl is low ,the only way to increase hdl is exercising ,no medication available can increase the level of hdl.
your triglycerides are on higher side ,researches show that indians and asian are at higher risk of increased triglyerides as compared to whites because of our genetics,
Medication like fenofibinates and statins are given to control increased levels of triglycerides , but medications are indicated only when your triglyceride level is more then 300mg /dl ,initally life style modification is tried for at least 3 months .
life style modification include-
strenous exercise for 45minutes a day for atleast 6days a week for 3months,by strenous it means heavy exercises .Practically heavy means something which makes you sweat profusely within 10minutes.
30 minutes of brisk walking daily,by brisk walking it means that speed should not be less then 5.1km/hr ,just walking or moving will not change anything it should be done properly ;minimum at a speed of 5.1km/hr .
Junk foods ,oily foods ,mustard oil ,ghee ,butter should be avoided at all cost.
Reduce your weight ,weight reduction is very important ,just do the exercises as i advised you above ,and with in three months you will see the changes.

Echo shows diastolic dysfunction ,don't worry as your weight will decrease the diastolic functions will also improve.
I would like to prescribe you only few medictions for your anxiety
Nexito should be taken for atleast two months ,this will help you to relieve your anxiety and panic attacks!
clonazepam (benzodiazepenes ) should be taken in low doses (it will keep your anxety levels in control) .
For doses of these drugs you should consult your physician

Embeta Xr 12.5 you could continue , it will help to improve your diastolic dysfunctions.
Beta blockers helps to relax the muscles of the heart ,hence allowing more blood to flow inside during diastole.

There is no need to take rosuvastatin, please discontinue it ,instead you can take PRE LIPID for 3months ,its a natural product containing flax seed and other herbs ,trial show that its very effective in controlling dyslipidemia , without the risks of side effects.

For spondylisis try to restrict the motions of your neck ,once your weight will decrease all your parameters will start to settle down to normal

Medicines can't cure you ,they can just help you !
Drastic life style modification is what you need at present ,just try whatever i said for 3 months and you will definitely feel much better!

In case you need any more help please do message me!

Thanks
