Question: Hello Dr, this is my wife's lab results please could you describe whats the possible problem with explanation (I am a pharmacist so you could explain it using medical terms etc.)

Hello,

your wife is having a high total cholesterol and LDL cholesterol but since she's young (I suppose) and does not have any cardiovascular risk factors (or you haven't mentioned any) there is no need to do anything more than watch her diet, body weight and physical activity. No drugs that is.

She's also having low iron in her body and needs supplementation. The most likely cause is blood loss caused by her periods or insufficient iron intake (low meat consumption). If these two conditions do not apply to her then investigation would be required to identify other potential causes. Ferritin measurement would help.

Finally, she's having elevated liver function tests. If she's overweight or obese then this is probably fatty liver disease (FLD). In any case an ultrasound scan of the liver is required. Retesting for transaminases and ALP is also required. Testing for various forms of hepatitis is essential (A, B, C and autoimmune forms).

Hello Dr, she had an ultrasound scan before and it showed that she has fatty liver disease but she is thin (weight: 53kg height:157cm) and she has some fat accumlation around her waist. She has low meat consumption, also she has heavy bleeding during menustration. Also, her recent pelvic ultrasound showed little distention in her gallblader (very little).

ALP is found in the image as Alk. Phos she has 127 iu/L (normal 26-88 iu/L) plus other LFT is shown in the immage.

Yes, I've seen the ALP but she has to repeat the tests to see if there are any differences which may mean that something is in progress (or not). FLD may occur also in high alcohol consumption and diabetes. Being overweight is another cause. Almost every obese person has FLD but many overweight ones ("some fat accumulation around the waist" usually means overweight) also have it.

Since she's loosing a lot of blood during menstruation, that's the most likely cause of iron deficiency. Iron supplementation should suffice.

Hey Dr. I've calculated her BMI she is 21 where is normal. so I'm not sure about the possible cause (she never drank alcohol, both of us (religious reasons)

21 is low... so she's not overweight although fat around the waist is a risk factor for FLD. FLD is a multifactorial disorder. I've already told you its most important causes but there are more. Genetics, other drugs, fast weight loss, insulin resistance, hyperlipidemia, etc. Sometimes it's not easy to identify one.

The worst thing about it is that there is no drug to treat it. A healthy lifestyle helps (healthy diet, physical activity and keeping the body weight close to normal). In any case I would recommend doing the suggested tests to exclude other more serious diagnoses.
