Brief Answer:
I have checked your reports.

Detailed Answer:
Hi welcome to Health care magic,
I have checked your reports and find the following--.
You have Pre Diabetes---strict diet plus an hour of walking is advised.
You have raised homocysteine--take folic acid supplements for this.
Your GFR is coming down-- It could be Diabetes or elevated blood pressure leading to diminished kidney functions.
Low D and B-12--Please take supplements for these.
If you are overweight reduce your weight.
Also I think you should do an abdominal sonography to look for fat accumulation in the liver.

Please discuss all these with your doctor.

You can revert back with further queries.
