Question: Is the infection related to the recent csec?
MSCT states: Evidence of loculated peripherally enhancing collecting in lower anterior abdominal wall below scar site. Evidence of loculated peripherally enhancing collections is seen in subcutaneous plane at lower abdomen at para median location extending to skin. It measures about 51 x 33 mm in size in axial plane, 55 X 43 mm in size in coronal plane and 60 x 46 mm in size in sazital plane.
OT notes: Pelvic and abd well abscess cavities explored and pus removed abd sent for culture. Dissection of teh pus cavity and adherent viscera. Large mass tubo ovarian on the R adnexa & on post aspect of uterus. Mass appears to be chocolate cyst.

Hello Dear. This infection is not related to Cesarean section. According the report it says that the mass is probably a chocolate cyst. I will tell you in detail what is a chocolate cyst. And since there is pus in the abdominal cavity it may be infected chocolate cyst.
A chocolate cyst is a sac or pouch growing on the ovary that contains fluids and sometimes semi-solid material too, in which case it may be referred to as a complex ovarian cyst. Because the endometrial tissue inside the cyst responds to monthly hormones it bleeds and fills the interior of these cysts with unclotted blood. Chocolate cysts get their name from the dark old blood that grossly resembles chocolate. Other names for chocolate cyst are endometrial cyst, endometrioma cyst, and chocolate ovarian cyst.
How do chocolate cysts develop? A women with the disease endometriosis will have tissue from the inside surface of her uterus (endometrium) attach to the ovary. These patches of endometrial cells may form small cysts that multiply into even more cysts when stimulated by menstrual hormones. As the cysts spread they can become part of the ovary itself, replacing the normal tissue needed for ovulation. Inflammation and irritation of nearby organs may result from chocolate cysts spreading around the pelvic cavity.
Treatment for chocolate cyst:
Typical options for chocolate cyst treatment include medication with drugs and surgery. Drugs may be helpful in relieving symptoms temporarily but do not treat or actually remove the cyst. Danzol and GnRH are two such drugs commonly used.
Now since there is infection and pus formation, pus is sent for culture & sensitivity. Once the report comes the doctor will come to know which are the organisms causing infection and which are the drugs which are sensitive and act against the organism. Accordingly the doctor will treat with antibiotics.
