What Does This Following Lab Report Indicate?

Question: Hi Experts,
This is my question for my mother who is having below symptoms.
1. Diabetes for 20 years.
2. Hypertension from almost 15 years.
3. Anemia average Hb : 8.5mg, this is also developed over a period of time
4. 8th months before discovered kidney problem (protein leakage)
5. Having severe pain in bones from last 5 years.

Currently she is getting diagnosis of first four symptoms and taking below medicine.
a. Humlog Mix 50 twice a day (20 - 0 - 10 units)
b. Minipress XL 5mg (1-0-1)
c. LN Bloc 10mg (1-0-1)
d. Seloken XL 50mg (1-0-0)
e. Inj. Crisp 25mcg once a week
f. Ferinject 100 mg once in a month
g. Cobadex CZ

I wanted to know on her Osteoporosis i.e fifth symptoms.
How well can we start her Inj. Zoledronic Acid 5mg once a year along with other medicines like calcium and Vitamin D3? Will it be fine to give her the injection by looking her current medicine and treatment going on?

For your reference Last lab (15 April) result (important one) are as below
1. Serum Creatinine: 2.4 mg/dl
2. Hb: 9 g/dl
3. Serum Urea: 87.5 mg/dl
4. HbA1C: 6.9%
5. Serum Uric Acid: 5.7 mg/dl
6. Lipid Profile: All OK
7. Iron Profile: All OK.
8. Serum Calcium: 8.7 mg/dl
9. Serum Phosphorus: 4.9mg/dl

Hope above information will be helpful to you to determine if we can start the treatment for her bone.

I am Dr Vaishalee and I am clinical pharmacologist, working for nephrologist. I will try my best to help you.

My suggestion is to avoid zoledronate as it is clearly contraindicated in kidney conditions.

There are other medicines that can be tried for osteoporosis and are safer like raloxifene, estrogen, calcitonin, thiazide diuretics. You can discuss with your doctor about these options and if these are right for her.

To help increase the overall health, lifestyle measures are also important like muscle strengthening under physiotherapist guidance and cleaner drinking water to avoid infections.

Hi Dr. Vaishalee,

Thanks for your advise Mam. Surely will check on your suggested medicine with the Doctor. For my understanding, is it possible to explain why and what made Zoledronic acid not suitable for Kidney Patient? How it effect and deteriorate the condition of kidney while other medicine does not?

Its probably due to direct cell damage

Hi again

Bisphosphonates in general and zoledronic acid specifically has been reported by doctors and patients worldwide to be causing this rare side effect.

The mechanism by which it causes kidney damage is not well understood. In most cases it only causes a transient (short-term) increase in sr creatinine levels. In some rare cases, especially on iv infusion, it can cause acute tubular necrosis (kidney failure). It seems that zoledronate enters the kidney tubule cells and damages it directly.
