What Does This Echocardiogram Test Report Indicate?

Question: I had an echocardiogram tte on November 24th. I didn't hear anything from my PCP like normal, so 3 days ago I went online to the clinic website and the results of the echo were there. On the dr's comments was the following: "So your pump function is good with the Ejection fraction being above 50%. (It's 60%) Your heart does not relax well though as you have diastolic heart failure. Needs to watch you sodium and fluid intake." I also have Other generalized ischemic cerebrovascular disease     09/12/2008; Occlusion and stenosis of carotid artery without mention of cerebral infarction 09/23/2008; Pain in limb 10/22/2008; Diabetes mellitus type II, uncontrolled 12/12/2014; Obesity (200 lbs) 03/20/2012; Stress 01/11/2013; Sleep apnea 02/22/2013; Skin inflammation     04/09/2015; Pustular psoriasis 08/14/2015; psoriac arthritis; Diastolic congestive heart failure, NYHA class 2 11/25/2015.
Several of these I was unware of. Saw the Neruologist Dec 9th and he tested and said I lose my balance due to diabitic nerve damage in my legs and feet. I am 64; support 7 people; pay all the bills; right now I'm working long hours as I got behind in the mortgage ($1800) per month. Extreme exhauston, my left shoulder and arm always ache & my chess always aches. I do get dizzy when I stand up quickly. My bood sugar A1c is 7.4 was over 9. I get severe charley horses in my legs at night. I have to stand up and force my heels down. In 1998 I had back surgery on L4&5.
My next PCP visit is in February. Shouldn't I see her before that? Should I be taking anything for the diastolic heart failure? I How much fluid should I be drinking? Have all but cut out salt. What else should I do?

Dear sir, Greetings and hats off to you, you know many medical terminology as well as how to deal with all the medical as well as surgical problems you have. My sincere advice is that reduce your stress first. If you need to keep your health in manageable condition you need to relax and may be work some time. Hope you are taking your medications for diabetes as well as heart problems. Life style changes are must. You need to reduce fluid and salt intake, better get evaluated further for your diastolic heart failure, it is not so serious for your age but still better meet your cardiologist for the proper advice. if you get swelling of your legs or giddiness/ chest tightness meet your consultant or else meet on scheduled date.
First stay away from all the fast / processed and fried food stuff. Divide your meal in to 4-5 parts per day whenever you feel hungry. With 2/3 snacks in between. That helps to keep the sugar levels constant throughout the day. Add more of fiber rich food stuffs like green leafy vegetables, raw fruits (not juices), take very less of carbohydrates (rice) avoid sugar completely. Take Millets in boiled form or the breads prepared out of it like ragi, bajra, sorghum, wheat etc., are very good source of micronutrients required for the action of insulin. Take more of protein rich food, like egg white, fish is good source of omega 3 & 6 fatty acids. Moreover, beetroot juice (inorganic nitrate), olive leaves, garlic, turmeric, tamarind also have proven benefits. Say no to oils and cholesterol rich food stuffs. Based on this, with the help of a dietician, you prepare your diet chart and strictly follow it.
Physical activity : walk at least half an hour to one hour a day. This not only helps you to burn calories, but also to relive your stress. Music is another way to relax too. Try and practice yoga, meditation and pranayama. Some aasanas are very easy to practice and also beneficial. Quit smoking (if) and reduce your alcohol intake.
Keep on track your glucose levels and meet your doctor for insulin dose. Take the medications properly and live stress free most of your problems are easily manageable
