What Does This Chest X Ray Indicate?

Question: Good evening. During recent Health checkup, Chest X-ray showed Small opacity in left mid lung.
CT Scan was done which states
- multiple calcified nodules in the bilateral upper and left lower lobes, likely due to prior granulomatous infection
- Multiple small nocalcified nodules in the left lower lobe, the possibility of active infectious etiology can not be excluded.
What should I do next? Thanks in advance.

Hi XXXXXXX What was your complaint that led to the Xray? Have you had Tuberculosis earlier? These findings are fairly common in people who have had treatment for Tuberculosis. Anyway, dont worry. I would also advice that you get a HRCT chest done so that we can know the situation better as an Xray is a very basic investigation. The Chest X-ray was a part of the Annual Health Check up. There was no issue reported, except sometimes a feeling of short of breathe after some activity or work.

General physician looked at the X-ray report and suggested a CT Scan.

I had mentioned the summary of City scan above, which has lead to some concern in mind. Whether it can be a tumor or anything else. Thus reached out to you for some clarification and advice. Especially the noncalcified nodules in the left lower lobe. I am providing the detailed CT Scan data below for your easy reference.

There was TB detected earlier (about in 1993) and was treated to cure. After 2 years of followup, Doctor mentioned everything ok, no need for further checkup.

With this background and the CT Scan report, please advice whether there is any concern and what may be the options now.

Here is the detailed findings of the CT Scan:

Lungs & Pleura: there is a 6mm subpleural calcified nodule in the left upper lobe. There are multiple small calcified nodules in the left lower lobe with mild fibrosis, atelectasis and pleural tickening. There are also multiple noncalcified small nodular opacities in the left lower lobe. There are calcified small nodules in the right upper lobe. No pleural effusion or Pneumothorax is seen.

Mediastinum: The central airway is unremarkable. The thoracic aorta is normal in caliber on this noncontrast study. There are multiple small calcified lymph nodes in the mediastinum and bilateral hila.

Bones and Chest wall: The visualized upper abdominal viscera are unremarkable.

Abdominal Viscera: The visualized upper abdominal viscera are unremarkable.

Impression:
Multiple calcified nodules in the bilateral upper and left lower lobes, likely due to prior granulomatous infection.
Multiple small noncalcified nodules in the left lower lobe, the possibility of active infectious etilogy can not be excluded. Recommend clinical and laboratory correlation.

Please help with your valued analysis and suggestions. Is there any level of concern here?

Hi XXXXXXX Thanks for the detailed report. It suggests some old and some recent infection. Chances of tumor or cancer are negligible. But I would still advice you to see a Respiratory Physician for completeness and surity. He can examine you and tell with certainty whether a biopsy is needed or not. I don't think it would be necessary in the light of old treated TB but its always better to be sure. Take care and let me know if I can be of any further help.
