What Does This Blood Report Indicate?

Question: Hi,
I recently got blood test for T3,T4, TSH, LH, FSH, Testosterone.
T3, T4, LH, FSH are in normal range. TSH is slightly higher (4.91)

However, Total Testosterone is around 115 ngdL
Free Testosterone is around 2.54 ng/dl
Bioavailable Testosterone is 63.41 ng/dL
SHBG is 22.18 nmol/L
Serum Albumin 4.6 gms%

I gave the blood for test around 2pm. I am very stressed out due to ongoing conflict with my wife. I also have not slept well for the last few days, due to some fights. My lifestyle is sedentary of late and I have not had any sexual intercourse in the last 8-9 months. I usually have 8-10 cups of coffee/tea, everyday. I use flakseeds, rice XXXXXXX oil in food everyday. My Vitamin D levels are also low and I am taking regular medication - hoping it'll catch-up if I walk in sunlight. I have acidity problems and take medication regularly.

I have had sexual intercourse for a year after my marriage.I still get erections, but not as frequently as I used to. My sex drive is quite low, of late, as I'm not even on talking terms with my wife and we're fighting in Family Court for a divorce.

Is this a permanent condition?
Can my testosterone levels be restored to normalcy by modifying lifestyle? What lifestyle, dietary changes can help restore testosterone levels?
How long should I wait, after modifying my lifestyle, to get another test?
Do I need therapy?

Your calculated free testosterone level comes to be 2.54 ng% which is very low Yes, it can be restored to normal levels by Hormone replacement therapy plus life style modifications
-You can try testosterone in gel form ( like androgel sachet) you have to tear the XXXXXXX and rub the whole contents on inner side of thigh daily after taking bath and drying. You should recheck Total Testosterone, SHBG and serum albumin after a month it may take 3-4 months for recovery
-You must start moderate exercise of minimum 200 minutes in a week - 8-12 weeks will give you result
-You can take some SSRI like Fluvoxamine 50 mg before going to bed
-Regular sex or masturbation helps to get back to normal testosterone level
-Vit D3 supplement in doses of 60000 units each week will also help
The medicines suggested are prescription medicines so need to be prescribed by a local doctor

I have a scheduled a penile doppler test in 10 days time. Should I wait for testosterone levels to increase or can I get the doppler test done as scheduled?
Will the results vary?

I read online that results will vary by 50% if tests are done in afternoon. I got the test done, empty stomach in afternoon. Should I get the tests done again, just so that I'm sure?

I used to masturbate daily, before marriage.
I get erections even now, during sleep and when I watch certain movies.
I cannot think about sex with my wife, as we can't even stand each other. Even if I have an erection and I think about my wife, I lose my erection. Is it because of lower levels?

Masturbation will help you maintain the testosterone level so definitely masturbation is not harmful. You can undergo doppler test to look for Vascular cause. No, don.t repeat just now, do it after 6 weeks. Don't worry about present erections because you can't expect good erection with anxiety provoking fantasy. You need to be patient. Don't rush for divorce. Many matters can be sorted out with time . Try to talk and address her concerns If not, then take your decision.
