Brief Answer:
Add a statin and ACE inhibitor to your therapy.

Detailed Answer:
Welcome and thank you for asking on HCM!
I have gone carefully through your query and understood your concern.

The target glucose level for your age is 80-120 mg/dl in the morning and up to 180 mg dl after meals. For your age the targets are a bit higher because trying to maintain low levels of glucose there is a great risk of hypoglycemia which may be dangerous. The HbA1C level should be <7.5%. If it is within this target it means that the actual therapy is the proper one. Continue with the physical activity.

Regarding the anti hypertensive therapy, instead of beta blockers i would suggest to take an ACE-inhibitor (Enalapril, Berlipril ect.) if your renal function is normal. It is very helpful in diabetic patients because has a renal protector effect. I would suggest also to take treatment to low the lipid level even if it is within the normal limits. It is very important in diabetic patients. Atorvastatin is recommended in every diabetic patient.

The annual control of the eyes, coronary, carotid and peripheral arteries to rule out chronic complications of diabetes is mandatory.

Hope the information will help.

I do not have type 2 Diabetes or am treated for it. My ACL is 5.8. I have tried other BP drugs and none were effective for the past 10 years. I went to a cardiologist friend and she put me on once a day Beta Blockers and it brought my pressure down to normal from 210/105. Aches and tiredness are the only side effects. I can live with that. I also have A-Fib and take a low dose aspirn for that for the last 15 years.I have a very strict diet and exercise program as I am not a big fan of drugs.Any input to that?

Brief Answer:
Continue with diet and physical activity.

Detailed Answer:
Welcome back!

Now the situation is a bit clear. The diagnosis of Diabetes is made when;
-Fasting glycemia is >126 mg/dl in 2 different measurements.
-Post prandial glycemia is >200 mg/ dl.
-HbA1c >6.5 %.

Postprandial levels 140-200 mg/dl is called impaired post prandial glucose tolerance.
Fasting levels from 110-125 mg/dl is called impaired Fasting glucose tolerance or Pre-diabetes.

You reported to have high glucose levels but as long as HbA1c is <7.5% (the target for your age) , is considered to be under control and no treatment is started. Just continue with the diet and physical activity and check HbA1C every 3 months.

Regarding the blood pressure, I still continue to suggest ACE inhibitors. You mentioned that you have tried different classes before, but ACE inhibitors in general do not have many side effects ( except of cough). If it is not possible to take it than continue with beta blockers as long as the profit is bigger than the side effects.

I would also recommend to check the thyroid function ( if you haven't done it) as you have AF and an over active thyroid should be excluded in these cases.

Hope the situation is much clear now.
Kind regards,
Dr.Mirjeta

Brief Answer:
Take care of your Prediabetes!

Detailed Answer:
You are welcome! Virtually may exist misunderstanding and the medical history should be very well prescribed. When you say that during the day levels are 175 mg/ ( not specified under treatment or not) dl the doctor imagines that you are diagnosed with Prediabetes at least. Anyway important is to conclude with clarified queries.
Wish you good health and all the best!

Dr.Mirjeta
