What Does This X Ray Report Indicate?

Question: Can somebody please tell me what does this mean??My mother is 68 years of age and this is the the x ray report. She recently had viral fever but is better now. She is having random coughs and that is why we did an x ray. the report is below.

Non homogeneous shadowing seen in right lower zone-suggests possibility of consolidation. Left costophrenic angle is blunted-suggestive of pleural thickening,the right costophernic angle appears normal. The left dome of diaphragm is elevated. Both hila are normal in size, have equal density and bear normal relationship. The cardiac size is normal. Aortic knuckle calcification is noted. Bony thorax is normal. Trachea is central in position and no mediastinal abnormality is visible.

Consolidation may be due to pneumonia, T.B etc.

Detailed Answer:
Hello XXXXXXX

Non homogeneous opacity in right lower zone suggests consolidation which indicates infection. It may be due to pneumonia,tuberculosis etc.
In such case, patient should be prescribed broad spectrum antibiotics empirically.
If it is non resolving then other possibilities like koch's/T.B should be ruled out. Malignancy is also one of the possibility in old age.

Left costophrenic angle is blunted and it is suggestive of pleural thickening. It is a benign finding and it is a healed stage. It is not a matter of concern.

Other findings like right CP angle, both hilum, cardiac size, trachea, mediastinum are normal.

Aortic knuckle calcification is age related finding.
Left dome of diaphragm may be elevated due to distended stomach.

Hello Doc,
My mother does not have any fever so can pneumonia be ruled out? Also can you suggest few anti biotics which I can give to my mother to cure the coughing? Also this coughing started when my mother had viral fever like attack. It stayed for 4 days. Now the fever is gone and she is much better but the cough has not gone completely. Can she take cough syrups? Also the coughs are white in colour.she throws them out. Also before she had viral fever she never had coughing or any complaints and was totally fit. So just wondering how can she have TB so suddenly? Please suggest.

Probably your mother is recovering,so her symptoms has improved.
Radiological findings take time to be normal.
She need proper evaluation.Since her symptoms has improved,expectorants can be prescribed after evaluation by a physician.

Pneumonia can be due to both bacteria or virus.There is no role of antibiotics in viral pneumonia.Patient only need symptomatic treatment and patient improve in 1-3 weeks.These are generally mild cases.
Where as bacterial pneumonia need antibiotics.
Blood culture and specific test for virus can be done after evaluation in serious patients.

Improvement in your mother is against tuberculosis.

I suggest you to give her plenty of fluids.Drinking plenty of fluids helps in loosening secretions and bring up phlegm.
Repeat chest x-ray(PA view) can be done after 10 days so that findings can be compared.Treatment depends accordingly.

Hello Doc,
Thank you for the reply. I showed my mother to our family physician. She has suggested Sputum AFB and culture test. Other than that she has prescribed AUGMENTIN DUO twice daily for 7 days. My mother seems to have fever just 1 time daily at 2-3 pm. Then after taking crocin it goes away for the whole day. She has just taken 1 tab of anti biotic now. I want to ask how much time is needed for the anti biotic to take effect and my mother to start feeling well again?

Sputum AFB and culture test is advised to exclude tuberculosis.
AUGMENTIN DUO is a broad spectrum antibiotic which can be empirically prescribed in consolidation.5- 7 days course of antibiotics is sufficient for bacterial infection.
After 2-3 days,antibiotic should start showing its effect.
