Question: dear sir
i have lost my weight 13 kgs from feb to till date and there is chills and sweating also.pet scan report that
FDG UPTAKE IS NOTED IN THE BILATERAL PALANTINE AND LINGUAL TONSILS
FAINT FDG UPTAKE IS NOTED IN BILATERAL SUBCENTIMETERS LEVEL1B CERVICAL LYMPH NODE --LIKELY INFLAMMATORY
NO DEFINITE PET EVIDENCE OF CLINICALLY SIGNIFICANT ABNORMAL HYPERMETABOLISIM
MY MONTUX WAS 15 AND DOCTORS STARTS THE ATT BUT THAT DID NOT RESPOND AND FEVER FROM MARCH TO XXXXXXX WAS THERE ,NOW WHAT I HAVE TO DO,PLEASE SUGGEST

Detailed Answer:
Hi,

Thank you for your query. I can understand your concerns.
It seems you are having PUO (pyrexia of unknown origin).
Mantoux test does not differentiate between current and past tuberculous infection.
TB gold also cannot distinguish between latent infection and active tuberculosis.
You have already been treated with empirical ATT but did not show desired response.
Since you are losing body weight with probably night sweating,alternative diagnosis to consider is malignancy particularly Lymphoma or connective tissue diseases or Brucellosis (Brucellosis almost invariably causes fever, which may be associated with profuse sweats, especially at night)
You should undergo screening test for connective tissue diseases like ANA(Antinuclear antibody test),ANCA(antineutrophilic cytoplasmic antibodies) ,Brucella agglutination test and a bone marrow biopsy apart from serology for HIV,for further work-up of PUO,at this stage.
