Question: Good morning Doctor, I need advice for treatment for my wife as she is having pain in the neck and under right shoulder for last 4 month. After consulting doctor and physiotherapy session for 2 months the pain did not go but increased. Yesterday she went for MRI and here is attached report.

Your wife's MRI report shows Bulging annular disc.

Treatment goes as below.

Conservative Treatment

A bulging disc does not necessarily mean that you will need to undergo surgery. The treatment of a bulging disc depends on your symptoms. Most patients with a bulging disc do not need surgery. Your symptoms can usually be managed with conservative treatment such as watching and waiting to see whether your symptoms go away, pain medications, and physical therapy. Many patients who initially have problems due to a bulging disc find their symptoms completely resolve over several weeks or months.

If your symptoms are getting steadily worse, your doctor may be more likely to suggest surgery.



The decision is left to your treating doctor and wife's clinical state.

Thank you so much for your kind advice. As mentioned earlier, this has been for almost 4 months now with pain medication and physical therapy for last 2 months. But the pain is not going away. I am not sure we should consult a neurologist, orthopaedics, a general physician, or a physiotherapist. Need guidance on what kind of physical therapy (and or exercise) will be suited for treatment.
