What Does This FSH Level Indicate?

Question: My wife is 47 years old. Her FSH count is 54.96. The TVS report states that her endometrial thickness is 5.4mm. There is cyst of dia 6.1 mm on her right ovaery. Further, there are two small fibroids in the posterior wall of uterus measuring 1.6 cm and 1.2 cm, another small fibroid of dia 11mm is noted anteriorly. Uterus measures 62x39x39mm. After about 8 months, suddenly she started having her periods from 18.8.14. At present it has come down. I am worried. Can anybody suggest? The gynecologist has prescribed DEVIRY 10mg 1 tab daily for 10 days to be continued for next six months.

Your wife's FSH level suggests that she is menopausal . If a woman doesn't have menses for st least six mths then she is in menopause . Bleeding that occurs after menopause may be abnormal . The reason for this could be either hormonal imbalance or a pathological cause . I don't recommend hormonal therapy like deviry before more tests are done . I would suggest that a endometrial topsy be done if further bleeding occurs . If this episode of bleeding is controlled then just wait nd watch . Further episodes of bleeding need management . The cysts and fibroids are too small to cause any problems . Starting hormonal therapy after just one episode of bleeding will necessitate continuous ling term treatment . This could be a solitary episode . If the bleeding stops i suggest wait nd watch .

Along with Deviry, the doctor has prescribed Oestrogel gel for external application on the thigh once a day after bath. Are there any chances of endometrial cancer since the thickness is 5.4 mm? Are you suggesting for enometrial biopsy if there is another occasion of bleeding? On this occasion, the bleeding started on 18th August and stopped on 22nd August.

HRT

Detailed Answer:
You have been given hormone medication as part of Hormone Replacement Therapy. This is something that needs to be continued long term. If you get dependant on it it will be difficult to wean you off of it. I normally dont recommend HRT unless the patient cannot bear the symptoms. In your case, there was a isolated episode of bleeding. I would wait and watch for further such episodes to start further management like HRT and biopsy.
If you have further episodes of bleeding, do a biopsy. With oestrogen gel, endometrial thickness can increase but since its given along with progestins, that can be avoided. However, i would suggest repeating another ultrasound after 6 mths.

Thanks for your views. So long both us had an active sex life. Even on 17th night we had unprotected sex (after a very long time) when she was a bit dry. Discharge was outside. Could it because of that bleeding had started? Can we continue with our sex life? Or shall we abstain from it? We always use condom.

Dryness can cause some discomfort and hence the chances of tissue damage are more . That can cause tears in the vaginal mucosa . No need to refrain from sex . But if further bleeding occurs after
Sex then she should be examined . Latex can also cause dryness . Make sure to use proper lubrication.
