What Does These Urine Test Results Indicate?

Question: Hi i post a question this morning about my duaghter who has blood in her urine. She went to the doctor and they tested her urine. Blood count is +++ and her white cell count in her urine is +++. How severe is this? The specialist can only see us at the end of the month, but i am considering going to another one as i dont know how serious it is. It is the second time this year she gets it, but never this severe. They did give her medicine

Brief Answer: Possibility of Urinary tract infection Detailed Answer: Hi Welcome to Healthcare-Magic Greetings of the day Dear Ms Presence of 3+ Blood and 3+ WBC/pus cells in Urine is quite significant. The cause and diagnosis of this will depend on clinical history and associated complaints. Most commonest cause of such laboratory value in this age group is likely to be due to Urinary tract infection( Cystitis), also remote possibility of Glomerulonephritis is to be considered. I would require her clinical history and other associated symptoms 1. Does she have other associated urinary complaints: Example: Increased frequency of urine, bed wetting, excessive crying while passing urine, foul smell or turbid urine 2. Does she have fever 3. Does she have puffiness of face 4. Was her Blood pressure normal during the examination with your Physician. 5. What was sequence of event during her first episode of blood in Urine. Was Ultrasound of abdomen or any investigation done at the 1st episode? it would be helpful if you could provide as detailed history and sequence of events surrounding these investigation. Hi dr thanks for your response yes the frequency of her urine is high, at a stage ( end last year) almost every 10 minutes, foul smell or rather a strong smell of urine, no bed weeting except last night but is was diarea. Its about the 5 time she has been diagnosed with it since she was born and she often cry to pass urine, sometimes she say it burns and mostly because she cant get the urine out ( feels like she wants to go to toilet but dont get it out). I dont see any puffiness of her face. No ultrasound has ever been done, she was merely requested to pass urine and then some kind of paper test was used totest her urine. Ussually ( now as well) the doctor give her antibotics Zinnat, 125 mg, 7 ml twice a day.i see each dose contains 5 ml cefuroxime. The dr gave her 100ml to complete, more knatraxil as she has a runny tummy also with blood and buscopan and reuterina tablets.

Brief Answer: Possibly Urinary tract infection Detailed Answer: Hi Dear Madam From the description provided by you , It is most likely to be Urinary tract infection. As the symptoms have been recurrent its important to thoroughly assess the urinary tract to rule out anatomical abnormality and also rule out Vesico-ureteric reflux as cause of symptoms. Apart from course of antibiotic provided and other medication, I would suggest you to consult Pediatric Urologist/Pediatric Surgeon to carry out following investigation in consultation with him/her 1. Urine Culture and antibiotic sensitivity 2. Ultrasound of Kidney, Ureter and bladder 3. Voiding Cystourethrogram(VCUG) 4. Blood pressure assessment In the meantime i would suggest you the following 1. Ensure good intake of fluids: It is easily possible by following Rule of '1'. I cup/glass of water 1 hour before and 1 hour after food. This will ensure adequate intake of water. This will ensure adequate urine output and will be helpful in flushing out infection. 2. Continue giving antibiotics 3. Citralka Syrup , one spoon added in glass of water will help difficulty in passing urine. Do get back to me if you need any further assistance anytime.
