Question: Hi, We recently took 2nd opinion regarding my husband's sperm count and the results are completely different from the 1st test. First test was very normal but the 2nd test after 4 months is very disturbing. Below are the recent test results: Request you to please kindly help us with your valuable opinion and what are the chances of having a healthy baby. I will be looking forward for your reply. Thanks a million. Brief Answer: Chances of chromosomal abnormalities exist Detailed Answer: Hi, Thanks for posting your query. The semen analysis of your husband, unfortunately, shows very few normal forms. The count is however, normal. There are a few facts I would like to explain here in this context. In asthenospermia, ICSI or intracytoplasmic sperm injection, a type of IVF procedure, is usually successful. When teratospermia or abnormal forms are associated with a normal count, chromosomal abnormalities are seen. When such a sperm is used for fertilization, chromosomal abnormalities are observed in the offspring. Thus, if this semen analysis is taken as the criterion, the ideal thing to do would be to go for donor semen. Before embarking on any major decision, I would prefer a consultation with an infertility specialist or XXXXXXX for a complete evaluation, repetition of the semen analysis and if needed, a testicular biopsy or genetic counselling. If there are any repairable causes, they may be looked into. Only after exploring all possibilities, please go ahead with a IUI or IVF with donor semen. Hi, Thanks for the reply. Going by the 1st Semen Analysis, which was very normal we started treatment for me. Took Siphene 50mg for 5 days starting with 2nd day of my period. And yesterday in the scan it was observed healthy & normal in size 2 follicles, today afternoon took HCG 5000 i.u injection and advised by the doctor to have intercourse after 36hrs. Now that the 2nd opinion semen analysis indicates ASTHENOTERATOZOOSPERMIA, if we still have a intercourse after 36 hrs, are there any chances of having a healthy baby or should we not go-ahead and try other means. Are there no chances of having a baby naturally or atleast by extracting the healthy sperm of my husband. Kindly Please help us with your reply. Thanks & Regards.

Brief Answer: Better to proceed with treatment after assessment Detailed Answer: Hi, Thanks for writing again. In the current scenario, I would suggest proceeding further only after a proper assessment of the state of the sperm. The risk of fertilization with an abnormal sperm exists if you proceed naturally taking this semen analysis report into consideration. Please discuss with your consultant also. Healthy sperm extraction is possible under the supervision of an expert in good facilities and you still have the chance of using ICSI, but I feel the chances of having a natural pregnancy with a healthy baby with such unhealthy sperm, are remote, I am sorry to say.
