Brief Answer:
UTI, Nephritis, Paraproteinuria are possibilities

Detailed Answer:
Thanks for posting on HealthCareMagic. I have gone carefully through the reports and details provided by you. I also noticed that the specific gravity is high, something that you have not mentioned. There are two broad possibilities. Firstly, the possibility that your doctor has considered is of urinary tract infection (UTI) for which he has prescribed antibiotics. You should try out this solution and see if it works, but I am not sure if it would be effective. The detail that you have not mentioned is that whether the bedwetting is of recent onset or whether it is going on till now. In case it is of recent onset, it is reasonable to assume the UTI. But if it has been going on and the investigation has revealed this, I would consider the second possibility where I would consider two separate conditions coexisting, i.e. bedwetting and the proteinuria.

I would request you for the following information first:
1. Onset and duration of betwetting
2. Onset of black ants on wet pants
3. His BP. If not measured already, please get his blood pressure checked and report it to me. Also mention if it has been measured using a pediatric cuff or an adult cuff.
4. His blood creatinine level. A urine creatinine level if done would have been further helpful
5. Mention the normal blood parameters too that have been evaluated
6. Any medicines that he is currently on

Based on the answers that you provide, I might ask you to go for a urine protein electrophoresis if I feel that could be required but it is hard to comment as of now.

I am not going into the probable causes as there are several and I do not want to bug you with big names. Let me mention that I am considering a condition called nephritis where the BP is also elevated. If not, I may consider conditions like Bence XXXXXXX proteinuria, Goodpasture syndrome or Hemolytic-uremic syndrome. I would come to the bedwetting separately but that too needs information from you.

I would wait for your response.

My son is bedwetting from birth till now 3 or 4 days in a week. if we awaken and he urinates at 02 hrs we again wet the bed on 4 am and again on 5. 30 am. Some times even in day time sleep. In toilet bowl ,Black Big Ants ( Makoda) are visible only in rainy season from 3 years(Including this one).B.p not measured by the doctor. other tests results will be done after 3-4 days. but please tell me that Nephritis and paraproteinuria are how much fatal and are complete curable or not. what will be the future life if these desease happens.will they affect the kidneys or the damage is already done? will the damage is cureable?

Brief Answer:
Two separate conditions

Detailed Answer:
Thanks for writing back. Based on the details that you have provided, I would like to assume the proteinuria as independent from bed wetting. Let me address them one by one.
Do not worry about the kidney causes that might be causing the proteinuria. They are generally treatable ones. Do not be worried until some critical diagnosis is made if at all.
Co.ing to the bed wetting issue. This is a problem that many parents face. Some simple measures can make a difference.
1. Limit the fluid intake before going to bed.
2. Design some simple reward systems XXXXXXX star scoring method wherein you give the child a star mark each dry night. When he scores a certain number of stars he gets some simple reward.
3. Wet alarms are quite effective. A moisture sensitive alarm is placed beneath the bed and it sounds as soon as the child starts emptying his bladder. This sudden setting off of the alarm makes the child automatically withold rest of the urine along with an awkward sensation and gradually he learns to control it.
4. Desmopressin and its analogues can be tried if all the above are ineffective.

Hope that helps. Revert back in case of further queries if any.
