What Does The Ultrasound Of Ovaries Indicate?

Question: Hi,
Can you help me make sense of this ultrasound report of my ovaries. I had one in 8/2014 which indicated right ovary with two clear cysts and left ovarty with a 37x34x30 mm cyst with a fluid level and uniform low level echoes. No abnormal color flow. Appearance most consistent with endometrioma.

I had a follow up ultrasound today with the following findings.

Right ovary clear cyst consistent with a follicle. Left ovary contains a 22x16x13 mm cyst with homogeneous low level echoes and a 31x27x30 with streaming low levels. Impressions again indicate an endometria.

A little about me, I take tamoxifen for breast care prevention and have multiple fibroids in my uterus. My dr. wants me to have a second opinion with a oncologist gynecologist. This frightens me. Is there anything in these reports that would indicate a referral to this type of dr.

I have carefully read your query and understand your concern.

The doctors in your case have rightly gone for the follow up due to the following reasons.

Since you are on tamoxifen therapy,there is always a possible risk of ovarian and endometrial cancer due to estrogen agonistic action on uterus and ovary but antagonistic action on breast(protective on breast,harmful on uterus and ovary).

Hence screening guidelines will always have high index of suspicion.

The features of right ovary are consistent with endometrioma.

The features of left ovary are complex radiological patterns reflecting cystic cancer like changes.

So these changes have to confirmed or ruled out since all complex presentations are not cancers and vice versa is true.

Hence you are referred for further oncological expert opinion.
