Brief Answer:
Get a confirmed diagnosis by FNAC or Biopsy.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Female / 47 years old- mammography done on one side - focal asymmetry of 1.3 cm and questioned area of distortion - requested for additional films with possible ultrasound - wants to know the meaning and how concerning it is.

The meaning:
There is a distorted area, indicating that this may be due cancer, fibrosis, fibrocystic disease like fibroadenosis. But this can be confirmed only by two investigations: 1) FNAC (fine needle aspiration cytology). 2) Biopsy.

This is concerning.

I would advise you to get ultrasonography guided FNAC.
If this is not confirmatory, lease go for biopsy as this is the only confirmatory tests and gives guidelines for further management.

I hope this answer helps you to get an early diagnosis and proper treatment.
