What Does The Following Stool Test Report Indicate?

Question: I need help with my functional my dyspepsia/dysbiosis/IBS.
I have done those tests:
- Ct head/stomach
- Camera both ways, several times.
- Barium food test (check for gastroparesis)
- Ultrasound
- Sibo test (showed low sibo)
- check if i had to much acid (not the PH heidelberg test, should i ask for it?)
- CDSA 2.0

I have been sick for two years now, where nausea is the main problem. I find relief when retching. Two days ago i felt horrible, i got depressed and felt everything was horrible, then i suddenly did a huge retch/burp that i felt through my entire stomach. After that i felt like i was 100% myself, and could do whatever i wanted without ANY nausea the rest of the day. I have just done a CDSA 2.0 test (comprehensive stool test), and need some help with the result.

What iam most curious about this test is:
- I have low DIVERSITY ASSOCIATION, why?
- I have low
"Products of Protein Breakdown (Total*) 1.0 L 1.8-9.9 micromol/g
(Valerate, Isobutyrate, Isovalerate)" Is this cause of Low stomach acid/enzymes?
- High "Fecal secretory IgA" (mine was 2669mcg/g), ive been on low fodmap diet for around 2 months now, iam not sure if it have helped much.
- How come i can go from being EXTREMLY bad, and then force myself to retch (sometimes i cant hold it back as well), and become much better? Sometimes i have to do it several times a day, sometimes i get a EXTREMELY "big" burp/retch to get up some sort of "gas" that makes the rest of my day be much better
- Is this gas in the gut that cant get out? If so, what can i do to remove it?

What i currently take as medicine: Remeron and surmontil low dosage, my doctor said it would help my stomach (used them for a year, still problems)
I just started taking Aciglumin, but will start on "NOW Betaine HCL" and maybe enzymes this week.

I hope i get a full good response, as i have met A LOT of doctors these two years..

Thanks for your query and an elucidate history.
To recapitulate:
Male/22 - Sick for 2 years - *Nausea is the main concern - *huge burp or retching gives 100 % relief - and then you are fine for the day - Wants to know he reasons and the way it can be cured -
In the hope- you must have met a lots of Doctors and tried all the possible medicines and see the list of investigations you have got done ranging from CT scan to CIBO and CDSA 2.0

Please post the reports on this link and write for Dr T Chandrakant, for me to study in details all the reports hence I can assist you more and answer all the questions you have; which probably may not be related to the present complaints of burping giving you relief- which is your main complaint.

As for the clinical picture you are presenting since the age of 20, now 22 my thoughts are as below:The commonest cause of the symptoms you have are:

Aerophagia: meaning swallowing the air, inadvertently- unknowingly and the reasons are usually a lot of stress and/or anxiety. It is a condition in which a person swallows lots of air while breathing and happens more so when he/she is tensed and the resultant air in the stomach collects in the fundus (topmost) portion of the stomach.
The distension of this particular area sends the signals to the brain and body in the form of nausea.
This nausea indicates the body to remove this trapped air out and you get all that discomfort.
Hence once you burp and remove the air from the fundus of the stomach, you start feeling fine till it is again filled with air.
This burp may be without any smell, I suppose and you must be getting immediate relief as if no problem was there.

Another probably reasons are:
Some foods due to intolerance.
Aerated drinks
Fast eating habits.
Multi-tasking, doing too many things at the same time.

We shall discuss all the other points after you post the reports, so that I can get the material to study your case in detail:

I would request you to give me the history in general, in details.
A good history is the only thing that can give the probably diagnosis and all the tests are basically to prove or to rule out a certain problem.

I was on holiday in crete for two years ago. Suddenly i went from being really good to really bad. I got nausea and much diahrea. I got antibiotcs and primperan. Nothing worked, and when i got home i felt some better, but then it got back again. I havent been on work for two years, i have also dropped out of my studies. The thing is that even when i relax, this need of retching may occur. Some times i get better after eating, some times i just get more nausea. Those times i feel nausea and a lot of gurgling in my stomach, food relief it most of the time. In the morning iam most nausea after eating. BUT, i also had some problems before my holiday. I had some days (not that often) i was bad on work as well. I know that my problem is cause physical. I went to a mental doctor which stated the same. I was one of the best sellers in my
Company, and i have always loved to meet new people/on seminars etc teaching other sellers. And those days ive been so lucky getting a HUGE retch, i have felt perfect the rest of the day... I just dont understand why i just cant get this huge one every day, cause i feel there is something that must get up, and when it happend i can feel it through my entire upper abdomen.

I can understand the whole things you went through- the holiday - diarrhea and nausea - antibiotics and primperon - better after reaching home - but got it again - ** off the work 2 years - dropped out of studies - need of retching may occur - nausea with gurgling in stomach - were the best seller in the company - consulted mental Doctor - huge retch made you perfect for the day -

All this history indicates that most probably the reason behind your retching must be hidden in the psychology.

Undergone all the reports you have provided:
Occult blood - negative
No ova or parasites seen.
Only potential pathogens seen- no pathogens seen.
Commensal means residence of colon bacteria report seen
Low products of protein breakdown indicated good digestion or low intake- nothing to worry about.
"Fecal secretory IgA" indicates you have good immunological response.
Low Diversity association will be corrected by probiotics and prebiotics, hence nothing to worry about.

The tests of CDSA 2.0: I do not find much of a clinical significance with the history of retching.

I feel that your joining the duties and/or studies may solve your problem as you will be engaged mentally and itself may be a relief.

I have visig a pahycolog which clearly state that it was not the problem. She said there was clearly something wrong in my stomach that affected
my brain, and not the other way around.. And no, i have used pro and prebiotics for over a year and my test still show low diversity association. Maybe there is a problem that wont let bacteria live in my stomach?

Some times i have loose stools and diaharrea. But it is the pressure in my stomach that "wants to get out" that is my main problem. Its like the food i eat ferment and get gassy. It was worse before i started low fodmap diet, but it is still here and i hate it. Sometimes im good in my stomach and i can do almost everything i want, and the next day i can become really horrible. Can low diversity make food gassy? That they cant propely diggest? What about my high secretory igA?

Noted that you have diarrhea and loose stools sometimes, but it is the pressure in your stomach (abdomen) that wants to get out.

I hope by this time you should have definitely noted the foods and the beverages that cause increase in these problems, you have to stop them one by one, see the results.

It is good that you have started low Fodmap diet. This is also called as IBS diet.
I would suggest a minor variation: please follow if you like as this helps all my patients and will helps you too.
I always advise my patients to write diets on 2 different papers, the one which always suit and the another list which occasionally suit or does not suit at all.
If you follow the diet charts you have made you will be more benefited.

It is good that you have done the best test in the world but it has again increased the doubts in your mind and fueled your problems.
All these tests are to be taken for correlation and are not the diagnosis in itself.
I hope you would agree.
The bacterial flora and all the parameters change every hour every day and hence can not be solely held responsible for your problems.

You are already taking pre and probiotics.
Your symptoms are a combination of many factors that we discussed. Low diversity alone can not be the reason.
You may please get review test for comparison.
Already discussed Secretory IgA.

Our main problem is Burping. Concentrate on that.
