I read your question and i understand your concern. I am sorry that you have to suffer from the pain via procedure. Ideally your doctor should have listened to your pain and must have anesthetized adequately or even sedated you so thatyou could have felt less pain. Usually periprostatic block is performed by placing a needle into the notch between the prostate and the seminal vesicle laterally. This is identified ultrasonographically as a white (hyperechoic) pyramidal area that the authors call the Mount XXXXXXX sign (because of its resemblance to a snowy mountain peak). Fat in this location makes the area easy to visualize on Transrectal ultrasound. I believe your doctor inadequately anesthetized you for which you had to feel the pain.
As far as bills and emergency room shift is concerned, that i can not comment on as i have no complete history of the procedure and periprocedural precautions. Paramedics asking questions like you mentioned indicate you were anesthetized and emergency room was to keep you till you recover.
