What Does The Following Liver Function Test Results Indicate?

Question: HI doctor can you please suggest me on my liver test is there something serious?
I am quite concerned about followings:
1: Creatinine 0.60 mg/dL
2: AST 64 U/L
3: ALT 122 U/L
4: ALP 227 U/L
5: Bilirubin Direct 0.23 mg/dL
6: Calcium
Note: I am 31 5'9'' and 106kg.My Bmi is 34.6
Update - Went to see my GP and he said he suspect Fatty liver 99%.He said rest is ok .Get ultrasound if want to b 100% sure but ultrasound appointments are not available for another 4 weeks.

Can i have Fatty liver without having elevated cholesterols as mine are
Total cholesterol - 189
Triglycerides 141
HDL 42
LDL 118.8
VLDL 28.20

Hep B and Hep C non reactive.No Fever no nothing.No pain in abs or anything.I just got my regular test done and seen these values .

I am Dr.R.K and I am pleased to assist you.
I presume you are a non-alcoholic.

The most common cause of this kind of liver function is non-alcoholic fatty liver disease (NAFLD).
As your doctor has said, you can have a ultrasound scan done to confirm it.
There are many causes for NAFLD - Obesity, diabetes, high cholesterol etc.
Your BMI is high indicating obesity.
So if you have NAFLD obesity is the most likely cause.
Weight reduction is the first step in the treatment of NAFLD.

You repeat your LFT after 4 weeks and also get an USG scan of the abdomen done.

Hi Dr,
Thanks for your reply.I have been going through a lot of mental stress coz of these liver Reports.I don't know why but i am super scarred of few things :

1:Hepatitis A,B,C
2:Liver Cancer
3:CirrHosis

Does my report indicates any of these.?

I know i have taken Hepatits B & C test ?are good enough to rule out any chance of B and C ? Do i indicate Hepatitis A ?i don't have no fever no nothing ?

I am really scarred to even get ultrasound done <what if any cancer or Hepatitis or something serious comes up there?

I Have taken fat burner called "ECA Stack" for two months during FEB MARCh time 2014.

I was anaemic and little week in 2013 but then in a week back to proper diet and i was fine but no fever no nothing then either.

I do feel something on right upper abs sometimes but no paid no nothing.

I am sorry for asking so many questions but since i got these reports i feel so useless and can not concentrate on work or anything.

i am worried and scarred and my anxiety level is high.

Note: i don't drink or smoke

Hep A test has not been done.
There is nothing suggestive of liver cirrhosis or cancer.
An ultrasound scan is the next step..

Got my ultrasound done and my
liver - mildly enlarged apace 16cms - & homogeneously cchogenic s/o fatty infiltration. Intrahepatic biliary radicals are not dilated. No Focal area of alterated echo texture/sol/cyst or abscess noted.
GB - All Normal
Pancreas - All normal
Spleen - Mildly enlarged in size app 13cms - Normal in shape & Echotexture.No focal area of altered echo texture seen. Splenic vein is normal in caliber.no splenic vein collaterals noted at hilum.

Kidneys - All normal
U bladder- All normal
Prostate - All normal
No free fluid seen in abdomen & Pelvis
No Enlarged para-aortic /parailaic lymph nodes seen
No Pl.effusion see IMP- Fatty liver & Mild Splenomegaly

Please suggest as spleen is swollen? again worried

Hi,
As per your report, you are having fatty liver. And yes there is mild enlargement of spleen. Portal hypertension is one of the causes. But it is less likely as there are no other features suggestive of it.
There are many different causes of spenomegaly - It could be due to infection, blood disorder or storage disorders.
A complete blood count and a peripheral smear test may be done first and further testing if needed like a bone marrow test may be planned based on the initial test reports.

Can this Mild Spleen enlargement be related to Fatty liver ?
Mild liver Enlargement - Does my US & LFT indicates Fatty Liver on XXXXXXX or NAFLD level.By diet and exercise no alcohol ,Can i reverse this ?

No, splenomegaly is not related to fatty liver. Fatty liver does not cause splenomegaly.

You are a probable steatohepatitis.
Yes you can reverse it.
As mentioned in my earlier post weight reduction is the first step.
