Brief Answer:
please see details.

Detailed Answer:
Dear Sir
I assume that all this measurements are in SI or standard international units.
1.The results show that there is high triglyceride levels and low HDL cholesterol levels. Uric acid is marginally high.
2. Common reasons for high triglyceride and low HDL combination are alcohol consumption, diet high in carbohydrates, diabetes, thyroid disorder, lack of exercise and obesity. As per the information provided, there is diabetes and possibly lack of exercise as there is severe reduction in ejection fraction. The recent guidelines stress on life style modification as the foremost approach to reduction in triglycerides. These include cessation of alcohol consumption, weight control, good control of diabetes, daily supervised exercise, correction of any thyroid disorder, consumption of fish and avoidance of too much reliance on carbohydrates. drug therapy for reduction of triglycerides is reserved for extreme cases only. That also has not been find to reduce mortality.
3. Reasons for this marginally high uric acid may be consumption of excessive proteins (like mutton / pulses/ chicken) or use of diuretics.
Hope this provide some insight into the issue. Feel free to discuss further.
Sincerely
Sukhvinder

Brief Answer:
please see details.

Detailed Answer:
Dear Sir
Whatever I enumerated were common issues. I also pointed out that diabetes and low exercise capacity are only evident causes in your case.
Many a times when no obvious cause is available after exhaustive work -up genetic causes are thought of for such high triglycerides. Medical fraternity is not unanimous about the bad of effect of triglyceride on heart. There is no consensus that they definitely increase mortality or death rate, that is the reason the drug therapy is not recommended for most of the cases. The drug therapy is given to prevent pancreatitis primarily rather than improving cardiac prognosis.
Hence, best way to treat this condition is to adopt whatever possible life style measures are there. Even after their adoption the levels continue to be higher than 5.7, you can consult your physician for assessment of need of drug therapy. Normally such a reassessment is done after 3 months.
Good diabetes control is a must for normal triglyceride levels.
Sincerely
Sukhvinder

Brief Answer:
OMACOR is specifically given to reduce triglycerides.

Detailed Answer:
Dear sir
1. Life style modifications include weight control, good control of diabetes, daily supervised exercise, correction of any thyroid disorder, consumption of fish and avoidance of too much reliance on carbohydrates.
2. Omacor is meant to reduce triglyceride levels. It is one of the foremost drugs beside "Fibrates" which are used in a small subset of patients. Diuretics and Nebil worsen the condition and increase triglyceride levels. Drugs like Galvus-met which improve diabetes control, indirectly improves the triglyceride levels.

Sukhvinder
