What Does The Following LFT Report Indicate?

Question: I am 46years old alcoholic with weakness, loss of appetite. My LFT shows:
1.Total bilirubin =3.3mg/dl
2.Direct bilirubin =1.0
3.Indirect bilirubin = 2.3
4.ALP =71u/l
5.Total proteins =7.1g/dl
6.Albumin =4.5g/l
7.AST =56u/l
8.ALT =90u/l
Kindly advise treatment

As per your query answers are-

You liver function are deranged.Bilirubin is 3.3 which is clinically significant and detailed investigation is to be done to rule out the cause of elevation.Initially i would like to give you an idea abiout liver enzymes and there role.
ALT is measured to see if the liver is damaged or diseased. Low levels of ALT are normally found in the blood. But when the liver is damaged or diseased, The cells in liver die causing the stored ALT to be released into blood causing ALT levels go up. AST is second enxyme used to detect liver damage however it is less specific then ALT. So it is often ordered in conjunction with ALT.As it is less sensitive for liver disease its significant only when both ALT and AST are elevated(for liver pathology).
Elevation in liver enzymes indicate inflammation or damage to cells in the liver. Inflamed or injured liver cells leak higher than normal amounts of liver enzymes, into the bloodstream, which can result in elevated liver enzymes on blood tests.
TYou are an alcoholic.
Most likely cause in your case can be -
Alcoholic hepatitis (severe liver inflammation caused by excessive alcohol consumption) or Alcoholic Cirrhosis (early stages of liver scarring).

Initially a liver ultrasound or CTscan should be done.However if picture is not clear even after these tests fibroscanning of liver along with biopsy is to be done.

Treatment can only be given after knowing the root cause of the problem.
Please answer-
Sexual history ?
Number of sex partners?
Number of partners of your partner?
Blood transfusion history?
Drugs history?
Kissing or sexual exposure to community sex worker etc in last 1-2 years?
(to rule out hepatitis b possibility).

Suggestion-
- Must stop drinking alcohol and never drink alcohol again. It's the only way of possibly reversing liver damage or preventing the disease from becoming worse.
-Silymarin is a mixture of flavonolignans extracted from the milk thistle .Its a liver rejunuvator and should be taken daily.

Folow up with reports.
