What Does The Following ECG Report Indicate?

Question: Hi Doctor,
Greetings for the day!!!
Due to chest pain because of Acidity for almost 1 month my doctor suggested me to go for an ECG, Stress Test & 2 D Echo.
My ECG was normal
Stress Test was Negative
Color Doppler & 2D Echo Repot:
Final Impression
1) Mitral Valve Prolapse (Grade I)
2) AML belly prolapse in Left Atrium - no mitral regurgitation
3) Visual Left Ventricular ejection fraction : 55%

Also
B-Mode
1) Mitral valve : Both mitral leaflets redundant, AML belly prolapse of 5.9 mm below the atrio ventricular valve plane in left atrium during systole, good leaflet tip co-optation
2) No regional wall motion abnormality, no chamber enlargement
3) Other valves normal, good left ventricular systolic function
4) Septae intact, normal sized pulmonary artery and branches
5) No pericardial effusion, no calcification, no vegetation, no clot.

Above are the result of my test. My doctor told me the heart is normal no need to worry, just wanted to cross check if this is normal or some issues would be there in coming future.
I am really worried.

Detailed Answer:
Hi, thanks for posting the concern in the HCM.
Your doctor was absolutely correct. There is nothing to worry.
Such pain, if at all comes from cardiac origin, would give positive stress test. As your stress test is negative you don't have to worry about any ischaemic heart disease.
The structural abnormality in your mitral valve is not of that much significance. It hasn't caused any functional problem like regurgitation.
Thirdly, your ejection fraction and wall motions are good.
So, overall from cardiac point of view, you don't have anything to worry.
Regarding your symptoms, it may be due to gastro-intestinal origin. If the problem persists, you may need to do the following tests-
Complete blood count, LFT, amylase, lipase
USG upper abdomen
Upper GI endoscopy
Stool for RE, ME, OBT
If everything is normal, and you still get the problems, it may be due to gastro-esophageal reflux disease.
Till you do all the tests you can have some prokinetic agent, like Levosulpiride or Domperidone one tab before breakfast if you have heartburn. As soon as you get the reports, you may report back to me directly or you may simply consult your local physician for further management.

Hi Doctor,

Thank you very much for your positive answer.

About the Pain, I am taking PANTOREC - D since 3 week as suggested by my Doctor, now a days the pain comes only in the morning at the middle of the chest and there is a lot of Burp happening daily and the pain rotates more towards the left side of the chest from upper stomach till middle of the chest.

My cholesterol test is normal, Sonography is normal, X Ray is normal, ESR is normal and blood count is normal.

Any diet u can suggest to get me normal from Acidity problem, I am suffering this since 2 yrs.

Also, just wanted to know about the grading of MVP.

Hi, thanks for the reply.
You are not at all boring me. I am very happy to give you all the answer.
1. Dietary advice-
i. Avoid fasting for more than 4 hours during the daytime.
ii. Plenty of oral fluid, preferably 3-4 lit/day
iii. High fibre diet and green leafy vegetables
iv. Oil not more than 2 tablespoons per day
v. Avoid junk foods, carbonated beverages, alcohol, smoking
vi. Take dinner at least an hour before dinner
vii. Avoid bulky meals
2. Your MVP is likely to be grade I.
3. Regular exercise for at least 30 min. per day should be started.
4. Continue your current medication.
5. You should get an upper GI endoscopy done.

Thank you for your advice & Diet plan, I shall follow the same and come back to you with the positive output soon.

About MVP, I just wanted to know will it effect me in future like if it goes to Grade II, Grade III or IV or it remains on Grade I for life time

I do have a Sinus problem due to dust, I sneeze a lot if get expose to dust

Just help me with above queries, I will close the thread and rate.

Hi,

Thanks for posting.

You have grade I MVP which is not that much of worry. But, the valve cusps changes with age, they lose their pliability and due to age related calcium deposition at old age they become rigid. So functionality is decreased. Along with that grade I may progress to higher grade with advancing age. But, with proper diet and lifestyle these changes are slow.

Secondly, regarding your upper respiratory problem, I think they are due to allergy, and varies with seasonal change.

You may try out the following -
Steam inhalation twice daily
Tab Montair LC one tab once daily at bedtime for 10days.

I think that would clear the problem.
