What Does My Transvaginal Ultrasound Scan Report Indicate?

Question: I am 31 years old, not on birth control, not pregnant, and had a sudden change in my period the past 3 months. It went from 5 heavy days to 3 VERY light days. Around the same time I would develop a slight burning sensation in my pelvic region which has since disappeared but now my right ovary hurts almost constantly along with lower back pain. If I move a certain way it feels like it is about to pop... strange I know. I also started getting horrible cramps during my period. I had a transvaginal ultrasound last month which showed a 2.3 cm Mildly complex cyst with septations and low internal echos. My PCP sent me to my gynecologist, which I saw yesterday, and after she reviewed the ultrasound she said she was certain it was a Corpus Luteum Cyst and would go away on its own in a few weeks. My question is.... I have had symptoms for 3 months with no resolve and the ovary and back pain is increasing instead of getting better. If what she says is true, shouldn't my symptoms be resolved after 3 months? Is this a tumor instead of a functional cyst? Thanks!

Hello and Welcome to Health Care magic Forum. Thanks for writing in to is with your Health query. I have gone through your query and understand your concerns completely. Corpus Luteal cysts can cause pain for 3 months , they have been shown to be cause of persistent abdominal pain for upto 6 months. Sometimes when there is bleeding within the cyst which can cause pain for longer durations. Since your Doctor is pretty sure it's a luteal cyst but Can you please upload the Sonogram report here at the forum so that I can also see it. Tumours donot cause pain as a presenting symptom specially the malignant ones .Regards Dr Manisha Jain OBGY Specialist.

PELVIC ULTRASOUND

HISTORY: Pelvic and perineal pain, 31 years old.

TECHNIQUE: Transvaginal pelvic ultrasound

COMPARISON: none.

RESULT:

UTERUS:
The uterus measures 7.6 x 4.2 x 3.1 cm. The endometrial echocomplex
measures 9 mm in thickness. No fibroids are seen.

RIGHT OVARY/ADNEXA:
The right ovary measures 4.1 x 3.4 x 3 cm. Mildly complex right ovarian
cyst measures 2.3 x 1.9 x 1.7 cm. This contains septations and low level
internal echoes. There are additional cysts and follicles in the right
ovary up to 1.1 cm in size.

LEFT OVARY/ADNEXA:
The left ovary measures 4.5 x 3 x 2 cm. Normal premenopausal appearance
with small cysts and follicles up to 1.1 cm in size.

Blood flow is seen in both ovaries. There is no free pelvic fluid
identified.

IMPRESSION:
There is a mildly complex 2.3 cm right ovarian cyst. This is typically a
physiologic cyst when the patient is premenopausal. Consider follow-up
in 2-3 months if this is felt to be clinically symptomatic.

Hello dear, sorry for the delay in response as I was struck in an emergency.
I have read the whole report and agree with the fact that most likely it is a corpus luteal cyst which is causing all these symptoms of pain in periods and scanty flow.
These symptoms are common but not worrisome usually.
Take tablet mefenemic acid during periods for the pain issue and wait for 3 months.
In case symptoms persist or worsens then a repeat ultrasound is advisable to see the existence and size of cyst.
