What Does My Thyroid Ultrasound Scan Report Indicate?
Question: THYROID ULTRASOUND FINDINGS: The right lobe of the thyroid measures 5.3x2.4x2.4 cm. The thyroid isthmus measures 0.4 cm. The left lobe of the thyroid measures 2.6x0.8x1.5 cm. There is a large, complex, solid, heterogeneous vascular nodule within the mid-pole of the right lobe measuring 3.0x2.1x1.7 cm. There is a mildly prominent lymph node within the right neck at the level of the submandibular gland measuring 1.8x0.9x0.7 cm, which has a mildly prominent cortex and a preserved fatty hilum. This may be reactive.
QUESTIONS: Please suggest the best immediate step (MRI or fine-needle aspiration for tissue diagnosis, or others)? What is the most gentle next step for 70 year old women?

A this point I think that MRI should be done since the nature of nodule can't be evaluated by ultrasound good enough.
This nodule has both characteristics of malignant and benign tumors, but since there is vascular component and there is also suspicious lymph node then needle biopsy may complicate and may be difficult to perform.
So if you have possiblity, it would be good to do MRI and scintigraphy which will define lesions more accurately so that biopsy wont be required at all.
If there are MRI characteristics of tumor then total thyroidectomy should be performed.
