Posted on Wed, 8 Jun 2016
Question: Sonography report shows my uterus bulky with 9.2×4.7×5.5 of size.Endomatrial thickness is 23mm. Bilateral overies are normal. No adnexal mass is seen. There is no free fluid in cul de sac.

My M. C. is disturbed for last one two years. I took treatment for it. It subsided for six months. But now it has again returned. I take almost 10 days to free.

What should i do?

Brief Answer:
You need hormone evaluation for management.

Detailed Answer:
Hello,

Thanks for trusting us with your health concern.

I would like to know a few more details- your past menstrual history, any pregnancy history and the outcomes, any recent weight changes, medication, illnesses, stress etc.

The sonogram report shows no adnexal pathology. Thus, with your menstrual history, and the age, it appears as though you may have entered the perimenopause, with exaggerated estrogen action resulting in endometrial thickness on the higher side and irregular periods, as ovulation becomes infrequent in peri-menopause.

If a complete hormone assay is done, which I would suggest, you can know the exact defect and try to get it rectified. I would also ask you to see a specialist for a proper clinical exam, a pap smear and plan further management based on the results. You would also need an endometrial biopsy to detect the pathology underlying the hyperplasia.

Meanwhile please try to maintain a healthy weight, adequate exercise and a healthy diet.

Hope your query has been answered. Please feel free to contact for further clarifications. I will be happy to help.

regards,
Shanti.V.

Brief Answer:
The investigations are needed.

Detailed Answer:
Hello,

Thanks for writing back.

I have gone through the reports. Most of them, except the latest sonogram report, were done two years ago. I also get that you were given hormone treatment two years ago. Since the hormone pattern fluctuates continuously, I would recommend a repeat investigation as suggested. T his makes it easier to plan further management.

Please get back if you need more suggestions. Hope this helps.

regards,
Shanti.V.
