default
Posted on Wed, 13 Jul 2016
Question: Hello Sir/Madam,
I am 39 years old. I have done pathological tests to check up diabetes and lipid profile. I have found the following result from the pathological report:
Glucose Fasting – 117 mg/dl
Glucose PP – 108 mg/dl
HbA1c – 6.10%
Cholesterol total – 180 mg/dl
HDL Cholesterol – 38 mg/dl
UDL Cholesterol – 99.20 mg/dl
Triglycerides – 214 mg/dl
Creatinine, serum – 0.98 mg/dl
Uric acid – 3.4 mg/dl

I am taking Februic 40 one tab daily as my Uric acid level checked in the last two quarters were 8 mg/dl and 7.5 mg/dl respectively. I have consulted with general physician and he has recommended the medicine.

Please advise whether I should take any medicine to lower triglycerides and stop Februic 40 as well. Please advise also whether I should take any medicine to control blood sugar level or controlling diet at present is sufficient.

I have checked FF glucouse in last quarters, I have found it around 122 mg/dl, 108 mg/dl respectively.

Any omega 3 suppliment that I can take to lower Triglycerides level please let em know as well.

Thank and reagrds,
XXXXXXX

Brief Answer:
Pre-diabetes needs life style modification.

Detailed Answer:
Hi,

Thanks for your question.
Noted your concern.

Fasting Blood sugar level of 100 to 125 denote Impaired Fasting Glucose (Classified as Pre-diabetes).
Also HbA1c 5.7 to 6.4 is classified as Pre-diabetes.
Its mean there is a high chance of progression to diabetes.

Considering your pre-diabetic status and deranged Lipid profile I may suggest-
1) Calories and fat restricted diet.
Usually I suggest 1800 kcal diabetic diet with 60 to 70 gram of proteins and less than 25 gram of fat per day.
Better to consult near by dietitian who can plan diet for you considering your current Body Mass Index and food habits.
2) Weight control if possible try to maintain healthy BMI (Body Mass Index) of less than 23 otherwise below 25.
3) Regular exercise: At least 150 min per week of moderate degree physical exercise may be advisable depending upon your tolerance level.
4) I may suggest use of Metformin which decreases insulin resistance (first line anti-diabetic medicine) if there is a family history of diabetes and/or BMI more than 30.

You can stop Tab Feburic as Uric acid is under control.

Do Fasting and postprandial blood sugar level, HbA1c, Lipid Profile and uric acid after 3 months of life style modification as suggested above, depending upon test results further management can be planned.

Discuss all this with your treating physician as physical examination is also important.

Hope this helps you.
