Posted on Mon, 3 Nov 2014
Question: My husband is 48 years old. His recent test results revealed having: C Reactive protein of 10.3 mg/l, SGOT 73 U/L, and SGPT 83 U/L. Other blood test results were normal. He is a bit overweight. He does not smoke or drink, or have any other medical problems. His thyroid test results were normal as well.
What could be the problem?

Brief Answer:
Fatty liver due to obesity.

Detailed Answer:
Hi,
Thanks for asking.
I am Dr. Jyothi BL and I will be answering your query.

Looking at the reports and query, my opinion is as follows:

1. The liver enzyme elevation is mild and possibly due to fatty liver disease due to overweight. (Non-alcoholic steato hepatitis - possible early stages). There is no active ongoing hepatitis, otherwise, the value would be in 300-400 range.

2. C-reactive protein - normal values can vary upto 10 mg/l. It is only mildly elevated. However, it indicates a high cardiac risk.

3. The problem appears to be overweight. Weight reduction with balanced nutritional diet and moderate exercises should improve overall values. It is still early stages and liver damage, is completely reversible.

Fatty liver due to obesity with mild inflammation is the problem at present.

Hope it helps.
Any further queries, happy to help again.

Thank you for your response. What does it mean "high cardiac risk"? Is there anything he can do to prevent this? He also has slightly elevated blood pressure. Can this be the reason for the increased C-reactive protein?

Brief Answer:
Elevated blood pressure alone does not.

Detailed Answer:
Hi Anila,
Thanks for asking again.

C-reactive protein or CRP indicates inflammation.
Inflammation of arteries is the one of the causes of atherosclerosis, which can cause narrowing of lumen and further ischemia and infarction.

Previous studies have revealed an increased cardiovascular risk. It is not an immediate risk. It reveals increased risk over the next one to two decades.

Elevated blood pressure, could cause minor CRP increase. However, there could be other causes in liver, which has caused enzyme elevation. Also check for lipid profile.
Try to control CRP levels through control of hypertension, weight reduction and nutritional diet (to improve XXXXXXX or non-alcoholic steatohepatitis)

If there was very specific cause, then CRP would not be a risk factor for cardiac. Until then action needs to be taken and followup done. Repeat CRP once again after following a strict regimen, in another three months.

Hope it helps.
