Question: Can I send you a Bodimetrics ECG record. Presently diagnosed with A Fib but now I see a large spike. Blood pressure normal. Age 76/male

The large spike which you can see in your ecg is called Premature Ventricular Contractions (PVC'S).The most common cause of PVC is myocardial ischemia,In case of blockage of coronary artery ,the blood supply to heart muscles decreases ,and PVCs can be observed in ecg.
Before going in details let me brief you what a PVC is-
A premature ventricular contraction (PVC) is a too-early heartbeat that originates in the ventricles(lower chamber of heart) and disrupts the heart's normal rhythm.Abnormal heart beat originates in atria (upper chamber) The pattern is a normal beat, an extra beat (the PVC), a slight pause,on ecg recording its stronger then a normal beat(large spike which you can see).The heart's regular rhythm is interrupted by a premature or early beat.
Medications such as digoxin (Lanoxin),given to control rhythm in atrial fib inpatient can also cause PVCs .So in case you are taking any medicine for rate or rythm control except blood thinner ,then please do notify me.As increased level of digoxin and some other drugs used in A-fib can also cause PVCs.
My advise
2d echo - to be done to see the functions of the heart, any compromised blood supply to any region of heart will be depicted in echo.
24 hour holter monitoring - its like ecg taken continuosly for 24 hours, it will depict the number of PVCs in 24 hour,and any other disturbance in heart rythm in 24 hours.
Serum electrolyte-Hypokalemia can also cause pvcs.
Serum digoxin levels-if you were on digoxin.
Kindly send details of any other medicines you are on.If a 2D echo has been done in past, then send to me the reports.

Attched is what drugs I am on. My GP came up with the same suggestion re Holder which I get next week. But here are all my drugs, most not relevant.

Hence i would advice you to get a serum potassium level done.If its normal;then lets wait for reports of holter and 2d echo.
