Question: My Mom had heavy bleeding last week, we visited doctor and they have advised to do biopsy and we have done that. the report shows "Sections studied shows very scanty cellularity with occasional endometrial glands that are disrupted, showing foci of nuclear atypia and pseudostratification, on a backfround of haemorrhage and fibrin material. no evidence of malignancy in the section." what does this mean. now doctor suggest to do uterus removal. please advise..

Brief Answer:
Please furnish relevant history.

Detailed Answer:
Hi,

Thanks for posting your query.

The biopsy report as such does not suggest any malignancy. However, there is some amount of cellular aberration.

I would like to know some more information.
--if your mother has attained menopause, if so, for how long
---if she has had any other significant medical history in the past
----if she is still menstruating, her menstrual pattern in recent times and if she is on any hormones
----her age
---any more investigations done..you can also upload reports

I will be able to give further opinion after I get this information. Hope to hear from you soon.

regards,
Shanti.V.

Brief Answer:
Needs further investigation.

Detailed Answer:
Hello,

Thanks for writing back.

Several factors have to be considered in postmenopausal bleeding. Endometrial polyps, endometrial atrophy, hyperplasia and lastly, endometrial cancer are the common reasons.

To localize the cause, a proper clinical evaluation, pap smear, trans-vaginal sonogram, endometrial biopsy or dilatation and curettage, hysteroscopy if needed are done.

The specimen that was sent for biopsy has been reported as unsatisfactory. Perhaps the sample should be retested. Please ask your consultant for further investigations also so as to help plan further management.

Hope your query has been answered. Please feel free to contact for further clarification. I will be happy to help.

regards,
Shanti.V.
