What Does My Abdominal Ultrasound Report Indicate?

Question: Hello dr
I would like to consult you about a case of one of my relatives
A 49 year old female with a medical history of mild Von Willebrand disease. Upon ultrasound, a cervical mass appeared (well defined fungating solid lesion with vascularity measuring 6.4 x 4.0 cm).

Abdominal ultrasound showed normal size liver, normal ovaries and normal bladder outline. Uterus appeared to be slightly enlarged and there was a lesion on the cervix (lobulated, low echogenicity measuring 4 x 3.7 cm)

MRI revealed a pelvic soft tissue mass in the pouch of XXXXXXX related to the posterior aspect of the uterine cervix (measures 5 x 4.5 x 6 cm and did not show significant enhancement following IV contrast). Ovaries appeared to be normal and uterus had a normal size and shape.
Different diagnosis (endometrioma and uterine tumor) and various treatment options were suggested

We would like to seek more opinions to be able to take a decision

Hello,
Thanks for the query to hcm,
A well defined fungating cervical solid mass with vascularity in a 49 years old lady indicates a cervical cancer with extension. Ideally a punch biopsy for histopathological examination should be done to confirm cervical cancer and stage the cancer.
Also tumour markers should be done especially.. CA125, CEA, BETA HCG , AFP.
Thirdly , if histopathology shows anaplasia and cancer, then a PET SCAN would be necessary to check for extension and spread and would be helpful in staging.
Endometrioma and uterine cancer is unlikely as endometriomas are usually attached to ovaries and uterine cancer presents with endometrial hyperplasia and atypia and not a mass behind the uterus . However an endometrial biopsy should also be done to rule out concomitant endometrial cancer.
